The Clinical Care Lead is an experienced practitioner who provides for the clinical needs of patients both directly and indirectly and acts as a clinical resource to the nursing staff and healthcare team. They are responsible for supporting the provision of effective and efficient delivery of patient care in a leadership capacity for patients, families and staff, under the supervision of the Clinical Manager for the program and in collaboration with the Clinical Manager of Patient Flow. The Clinical Care Leader is responsible for coordinating patient care throughout the hospital stay and facilitating a safe and effective discharge process. This role requires collaboration with multidisciplinary teams to assess patient needs, develop care plans, and ensure continuity of care post-discharge.

REQUIREMENTS:

  • Current certificate of competence as Registered Nurse (RN) from the College of Nurses of Ontario.
  • BScN. or RN with equivalent relevant and recent preparation/courses.
  • Basic Cardiac Life Support (BCLS) Certification
  • Minimum of five (5) years’ experience in medical or related programs, or community experience in relevant areas such as case management or care coordination
  • Experience in a leadership role in a related nursing field is preferred.
  • Excellent interpersonal and conflict resolution skills
  • Excellent verbal and written communication skills
  • Knowledge of Renfrew County and community resources
  • Excellent organizational skills and ability to set priorities
  • Ability to work collaboratively as a team member
  • Ability to demonstrate leadership
  • Ability to coach, mentor and influence
  • Proficiency in the use of computer applications and software programs
  • Must be flexible and adaptable in a continually changing environment
  • Embodies the LEAN ideology by actively and continually seeking opportunities for continuous improvement.
  • Must have demonstrated ability to meet the attendance standards of the Hospital
  • Bilingualism preferred (French/English)
