Job Description:
As a member of the Clinical Resource Team (CRT) you will have the opportunity to work across programs and across Trillium Health Partners sites. The CRT staff is a vital component of ensuring uninterrupted patient care delivery by providing coverage for planned absences of inpatient Occupational Therapists. This position requires an individual who is highly skilled, agile and adaptable with excellent time management and interprofessional skills. Within the CRT Operations and Practice are supported by a Manager, Educator and the Professional Practice Leader for Occupational Therapy. The Occupational Therapist is accountable for the following:
- Assess patients with a holistic perspective: assess physical, psychological, perceptual, cognitive, vocational and functional status through administration of formal and informal assessments for each patient.
- Analyze findings to establish patient-centered goals, in collaboration with the patient, families, and interprofessional care team.
- Implement a patient-centered plan of care to facilitate/improve daily function
- Evaluate progress and goal attainment in the areas identified during the assessment and care planning process. Evaluate the need for supplementary assessments, referrals, equipment/adaptive devices, community services, and education.
- Participate in education and discharge planning in collaboration with their team and family to determine patients' functional status and level of independence to facilitate their safe discharge.
- Maintain assessment reports and progress notes in the patient's health record.
- Collaborate and communicate with interprofessional team and other stakeholder(s) within the circle of care.
- Assign and supervise occupational therapy related activities, as appropriate, to Occupational Therapist Assistants (OTA) Participate in the education of health professions students
Qualifications:
- Candidate must be a graduate of an accredited School of Occupational Therapy (Master's degree or Bachelor’s Degree in Occupational Therapy.)
- Member in good standing with the College of Occupational Therapists of Ontario
- Membership in O.S.O.T. and/or C.A.O.T. preferred
- Experience treating acute care Adult populations (Medical, Surgical, Neurological, Cardiac Oncology, Palliative and Geriatrics)
- Good listening, observation, organization and communication skills are essential.
- The ability to work independently, learn quickly and set priorities.
- Ability to work collaboratively with an inter-professional team Satisfactory attendance and performance record required
