About the role
The Emergency Department at St. Michael's Hospital is currently looking to hire a Community Resource Staff who will work in the Rotary Centre. Key components of the position are to provide a positive space for our vulnerable patients as they transition from the emergency department back into the community. There is a strong emphasis on strengthening the link between community agencies and the Emergency Department.
RESPONSIBILITIES:
- Providing respectful meaningful care for vulnerable populations using the rotary centre for respite from circumstances.
- Ensuring that the unique needs and living circumstances of the client are factored into the care, communication and discharge plan;
- Reviewing care plans and discharge plans to assess the clients capability for implementation in light of available resources and supports;
- Exchanging and sharing information with the community care giver about the visit and discharge plan;
- Identifying barriers to access or obstacles to optimal care and making recommendations to management as appropriate.
- Collaborate with other members of the health care team (Rotary Centre Staff, Emergency Department social worker, peer support worker, Registered nurse, Physician, SMH inpatient/outpatient units) to advocate for and participate in safe discharge planning and care for patients.
QUALIFICATIONS:
- Community Support Worker certificate or equivalent
- Sound understanding of inner city populations and community issues.
- Understanding of a Harm reduction philosophy of care
- Proven experience in developing supportive relationships with homeless and socially isolated clients.
- Good judgement in identifying crises and proven ability to diffuse and de-escalate conflict. Non-violent Crisis Intervention training an asset
- Excellent communication and interpersonal skills to liaise with clinical staff as necessary.
- Ability to independently manage a variety of competing priorities and demands in a high-traffic environment with multiple interruptions.
- Proficient computer skills with advanced knowledge of word processing, spreadsheet, graphics software i.e. Microsoft Word, Excel and, PowerPoint.
- Internet experience.
- Certification in Basic Life Support preferred.
- Excellent communication, problem solving and team collaboration skills.
- Must have an acceptable attendance record.
