The Opportunity: The Clinical Nurse Liaison (CNL) is an integral part of the Clinic Solutions Team, responsible for the coordination and delivery of services related to patient care offered by Sentrex Health Solutions. In this role, the individual is to act as a single point of contact for physicians, clinic nurses, and patients. This role includes customized administrative and clinical support for physicians, frequently within their offices, as the CNL takes full ownership of patient onboarding and supports the ongoing needs of patients and HCPs throughout the entire treatment journey. The CNL is responsible for ensuring optimal coordination of care between HCPs, patient support programs and pharmacy, ensuring patients are provided easy access to their prescribed therapy. This role involves direct interactions with patients, insurance companies, provincial drug programs, pharmacies, and other health care professionals (HCPs) to explore and obtain coverage for prescribed medications. The Clinical Nurse Liaison may provide other services as requested by their affiliated physicians, such as injection training and patient education on disease state and medication.
As a float CNL, we are seeking a flexible, reliable, and experienced Nurse to join our team. This nurse will be cross-trained and utilized across various clinics to support clinical operations and ensure seamless patient care. As a Float Nurse, your primary responsibility will be to provide coverage for staff vacations, sick time, additional support on current and ongoing projects, assisting Therapeutic area lead with training and onboarding on new staff.
A Day in the Life (What you will do here):
- Perform HCP and patient registration activities, according to protocols and applicable departmental procedures.
- Serve as a flexible resource across multiple departments/units based on staffing needs.
- Provide coverage for staff absences, including scheduled vacations, sick days, and other leaves.
- Offer relief coverage during peak hours, high patient volume, or unplanned staffing shortages.
- Quickly adapt to different workflows, documentation systems, and care protocols across clinics
- Deliver high-quality, patient-centered care with minimal supervision, even in unfamiliar settings.
- Process data entry activities of pertinent information received at different points of contact during the delivery of services.
- Provide medical support to patients and HCPs, including but not limited to education on inflammatory bowel disease (ulcerative colitis and Crohn’s disease), advanced drug therapies, and disease management.
- Assist the physician with the completion of all patient support program‐specific paperwork and follow‐up on submissions pertaining to obtaining drug reimbursement/coverage through private and provincial drug formularies to ensure patients have access to their prescribed treatments in a timely manner.
- Act as a resource to physicians who are prescribing this medication and continuous support for patients for on‐going support, renewals, and coverage changes.
- Facilitate communication with physicians, Patient Support Programs, or other health care professionals involved in the patient’s circle of care
- Conduct chart reviews to obtain data required required for special authorization forms
- Coordinate medication fulfillment with Sentrex Pharmacy
- Prepare for and attend extensive, specialized training sessions for specific therapies.
- In person and virtual support (including adherence support both inbound and outbound as needed).
- Review patient files prior to treatment to capture medical history, organize and review pre biologic work up
- Document and maintain patient records to ensure treatment history is adequately captured.
- Maintains confidentiality of patient and corporate information and discusses same only with appropriate personnel.
- Liaise with physicians’ offices, pharmacies, services providers, and other parties to provide comprehensive and coordinated service to patients and HCPs.
- Participates in ongoing internal and/or external continuing education activities.
- Administers medications (IM and SC) in the patient’s home or specialty clinic settings as required, as well as provides virtual oversight to remote patient groups.
- Monitors patients post injection, and documents on all activities completed.
- Hosts patient education based on specific patient needs, individually or in group settings.
- Responsible for scheduling appointments.
- Participates in telephone consultations and virtual meetings/training sessions as required.
- Adhere to professional nursing standards at all times.
- Work collaboratively within multidisciplinary health care team providing oversight.
- Proactively offer support and advocacy to patients and HCP which includes but is not limited to enrollment, facilitation of coverage renewals, and changes throughout the patient journey, as well as facilitating reimbursement support to physicians and patients.
- Provide ongoing support on disease state or product related questions.
- Read and review all Canadian product monographs to ensure a comprehensive understanding of each product being administered.
- Report and document Adverse Events as per training per Pharmacovigilance requirements.
- Adhere to Sentrex policies and procedures.
- Assist in the development and adherence to forms, policies, and work instructions, to ensure the safe handling, preparation, and administration of product.
- Follow and abide by all provincial College of Nurses guidelines while providing patient care.
- Additional duties as assigned by the manager.
What you need to ensure you are set up for success:
- Minimum 2-4 years of clinical experience in GI/IBD (preferred)
- Licensed RN or RPN
- CPR and First-Aid certification required
- ACLS certificate required
- Must hold a valid and unrestricted driver’s license
- Able to confidently deliver education including but not limited to patient self-injection education
- Ability to confidently administer medication IM and or SC
- Experience in a critical care, emergency and acute care as strong asset
- Confident and experienced with using Excel, Outlook and Word
- Able to work autonomously with little supervision, as part of a larger remote team
- Able to travel within a set geographical boundary
- Experience in Patient Support Programs is an strong asset
- Experience in Hepatology, Immunology, Oncology, Gastroenterology or Rheumatology a strong asset
- Understanding of the pharmaceutical industry an asset
- Strong analytical and mathematical skills
- Effective interpersonal skills
- Strong organizational skills and attention to detail
- Ability to meet deadlines and be flexible with patient and HCP requirements
- Excellent problem-solving skills; ability to resolve issues effectively and efficiently
