Right at Home Canada Fraser Valley is looking for a Care Planner & Community Relations Representative for our Abbotsford office. We are a private home care company serving the Fraser Valley (Abbotsford, Langley, Chilliwack, Mission and surrounding communities). This hybrid role blends C are Planning with Community Engagement . You’ll guide families through care solutions while also representing Right at Home in the local community, building strong referral partnerships and driving awareness. You will lead the development and oversight of personalized care plans for our clients, from initial assessment, planning through ongoing monitoring and quality assurance. In this role, you’ll serve as the main point of contact for clients, their families, and our caregiving team, ensuring open communication and peace of mind for all involved.

Key Responsibilities Care Planning

  • Conduct initial in-home consultations with new clients and their families to assess care needs, preferences, goals, and home safe considerations.
  • Develop comprehensive, individualized care plans addressing each client’s health needs, daily living support and personal goals.
  • Coordinate the startup of services and caregiver introductions for new clients.
  • Work closely with the scheduling/operations team to ensure care coverage and smooth service delivery, including managing any schedule changes or urgent care needs.
  • Provide ongoing coaching, set clear care expectations, and ensure caregivers follow established care plans and company standards.
  • Facilitate training refreshers or resources for caregivers as needed to uphold excellence in care.
  • Ensure all records are accurate and completed in a timely manner, in compliance with company policies and BC health regulations.
  • Document care plans, assessments, progress notes, and all significant interactions or interventions in our care management software (AlayaCare)
  • Facilitate targeted action plans to resolve issues, escalations, and maintain client satisfaction.
  • Track key indicators (inquiries, client satisfaction, caregiver feedback, etc.) and recommend improvements to enhance the client experience.

Community Relations

  • Build referral relationships with healthcare providers, organizations, and senior-serving professionals.
  • Conduct 15+ outreach meetings/month to drive awareness and client inquiries.
  • Create, administer, and implement winning business development strategies.
  • Help shape the company’s long-term objectives and determine plans for how to meet them.
  • Meet monthly key performance indicators (KPIs) tied to outreach, referrals, and client acquisition.
  • Comply with growth process standards including managing growth pipeline to ensure timely and accurate forecasting, tracking, and reporting sales activity in CRM tool and maintaining accurate records of all sales activities
  • Document all activities whether directly with referral sources/Influencers or with Care Offices into our client management program, with complete notes and follow-ups clearly and thoroughly input.
  • Monitor relevant community events for community-awareness opportunities and participate as appropriate.

Qualifications & Requirements

  • Background in healthcare, case management, or social work (LPN, MSW, OT/PT, or equivalent).
  • Strategic thinking and planning: ability to identify potential partnerships, evaluate their value, and develop a plan to establish and grow the partnership.
  • Play an active role in requests for proposals (RFPs) from potential partners and customers
  • Relationship building strong interpersonal skills, ability to build and maintain strong relationships with partners and internal teams.
  • Negotiation: strong negotiation skills, ability to effectively negotiate mutually beneficial agreements with partners.
  • Networking: extensive network in relevant industries, ability to make connections and build relationships with potential partners.
  • Market and industry knowledge: comprehensive understanding of the market and industry trends, ability to identify new partnership opportunities.
  • Comfort with tech tools (MS Office, CRM platforms) and willingness to travel locally.
