CommunitiCare Health is the first agency in Toronto to offer a full range of community health care services in a single, publicly funded organization. We bring together primary health care, community support services for seniors and individuals with disabilities, home care, and community mental health and addictions services, including supportive housing. Serving West Toronto, our mission is to keep our clients mentally and physically well, living at home, regardless of where they call home.
Today, CommunitiCare Health employs 400 staff across four main locations, including two clinical sites (with a third opening in fall 2026) and two sites supporting our Home Care & Community Supports and Mental Health & Addictions services. In addition, our supportive housing programs are located throughout Toronto’s west end, and many of our teams provide care directly in the community, meeting clients where they live.
We are currently looking for new members to join our Board of Directors. As a governance and skills-based board, you will bring your professional and lived experiences to support our bold strategic plan to build a stronger, integrated community health care system that better serves our community members.
We are looking for candidates with professional and/or lived experiences in the following areas:

Law, legal governance, compliance, or regulatory affairs
-
Capital projects, facilities management, real estate development, or infrastructure financing
-
Community based health services, supportive housing or integrated care models
The ideal candidates will be ready to join a collaborative team that is dedicated to contributing to the community. You will have the personal integrity, initiative and curiosity to actively engage in group discussions. While previous board experience is not required, you bring a strong willingness to learn and be ready to build your leadership skills while helping others become stronger leaders.
