Position Overview

The Case Coordinator Specialist applies professional knowledge and expertise in the assessment, planning, implementation and evaluation of supports to Case Coordinators and other staff in the Home Care program. Primary responsibilities include education, orientation, training and quality control to ensure competency and consistency in the use of electronic record systems and assessment tools, provincially and within Southern Health-Santé Sud.

The incumbent exercises the appropriate level of initiative and independent judgment in determining work priorities, work methods to be employed and action to be taken on unusual matters. The position functions in a manner that is consistent with the mission, vision and values; and the policies of Southern Health-Santé Sud.

Experience

  • Minimum Three (3) years’ experience as a Home Care Case Coordinator.
  • Experience in adult education and training an asset.

Education (Degree/Diploma/Certificate)

  • BSW, OT, PT, BSc PN, RPN, BN, RN, SLP, RD or related health/human service degree required.
  • Combination of education and experience may be considered.

Certification/Licensure/Registration

  • Active license and registration to practice as required by professional association.
  • Gerontological Nurse Certification (C) an asset.

Qualifications and Skills

  • Knowledge of Home Care systems and programs.
  • Knowledge of Provincial Home Care Services and Policies.
  • Knowledge with Electronic Home Care Record and Assessment Tool (e.g. InterRAI-HC).
  • Knowledge of relevant legislation and standards.
  • Demonstrated ability to lead and mentor teams and individuals.
  • Demonstrated effective oral and written communication skills through clear, concise, and professional interactions across various formats and settings.
  • Demonstrated effective decision-making and problem-solving skills through innovation and forward thinking.
  • Demomstrated organizational and time management skills through successful prioritization of tasks and efficient handling of multiple responsibilities.
  • Demonstrated ability to build and maintain professional working relationships with management and staff within the region as well as appropriate outside vendors and agencies.
  • Demonstrated ability to work both independently and as part of a team.
  • Given the cultural diversity of our region, the ability to respect and promote a culturally diverse population is required.
  • Proficiency of both official languages is essential for target and designated bilingual positions.
  • Demonstrated ability to respect confidentiality including paper, electronic formats and other mediums.
  • Good work and attendance record.
  • Requires a valid Class 5 driver's license, and all-purpose insured vehicle and liability of insurance of at least $1,000,000.00.

Physical Requirements

  • Demonstrated ability to meet the physical and mental demands of the job.
  • No hazardous or significantly unpleasant conditions.
  • May work occasionally evenings and weekends as necessary.
  • Will be required to travel to other regional facilities as the position duties may require.
