About the role
Summary of Duties
K2W is a complex medical / transitional care unit within the Veterans Centre where a primary nursing care delivery model of careis incorporated. The RN is responsible for the total care which includes personal care (ADL’s) of patients. The successful candidate will have experience working with a wide variety of medical needs of community patients. This would include Dialysis, Dementia, Palliation, TBI, Oncological, complex medical needs.
The RN supports the Philosophy of Patient-Focused Care concept and principles and the Mission and Values of Sunnybrook Health Sciences Centre. The RN works closely with other members of the multidisciplinary team and can demonstrate their approach to excellent patient centred care.
Qualifications/Skills
- Current Ontario Registration
- Current B.C.L.S.
- Gentle Persuasive Approach (GPA) or PEICES certification considered an asset.
- Dialysis certificate preferred or in progress (Hemodialysis, peritoneal dialysis)
- Oncology experience preferred (chemotherapy)
- Palliative care experience preferred.
- Strong conflict resolution and problem-solving skills required. This includes strong communication skills with the multidisciplinary team, patient family members and/or primary care givers.
- Strong physical assessment skills required.
- Ability to function in an interdisciplinary setting as a primary nurse.
- Excellent assessment skills
- Team-player
- Airway Management (Tracheostomy, CPAP, BiPAP)
- Central Venous Catheters (CVC) e.g. Hickman, PICC
