Job Description

The Psychogeriatric Resource Consultant (PRC) provides educational and consultation services to front line staff that care for seniors with behavioral disorders associated with cognitive impairment (BPSD). The mandate of the PRC is to provide behaviour management focused education, consultation and support to staff in Home and Community Care Support Services. Consultation includes education and intervention strategies for staff who care for individuals with significant behavioural challenges associated with progressive cognitive impairment. The three main pillars of this work will be to (1) provide system coordination and management; (2) provide integrated service delivery and (3) to ensure knowledgeable care teams to enable capacity building.

The PRC will be an employee of William Osler Health System and the position is aligned with Osler's Specialized Geriatric Service Outreach Team. The PRC will be mobile with most of the direct work occurring in Home and Community Care Support Services/Long Term Care Homes in the Central West geography. The PRC will provide supportive educational consultation and support services and contribute to quality improvement initiatives within the organizations that are serviced. S/he is expected to play a leadership role in promoting and supporting evidence-based care with seniors with behavioural challenges associated with dementia and other cognitive disorders.

Accountabilities:

  • Assist and advise in the development of care plans and interventions for persons with responsive behaviours and personal expressions

  • Uses solution-focused principles to motivate form care partners to carry out behavior management/intervention plan(s) that has been developed advocating for older adults quality of life and minimizing risk

  • Assists and supports staff in various sectors in the application and interpretation of assessment tools for care planning and treatment for persons with cognitive/mental health needs and associated behavioural issues for capacity building

  • Collaborates with other specialists, consultants, and community teams to improve the quality of life for the client and their families

  • Identifies knowledge and skill gaps/needs of community and staff in diverse organizations and settings who provide care and service

  • Builds capacity with staff and leaders utilizing a knowledge-to-practice framework such as PIECES and GPA as well as supports in the development of training materials and presentations

  • Identifies training/capacity building opportunities and delivers time sensitive education to care partners

  • Promotes smooth transitions for older adults across the continuum of care and develops linkages with community resources

  • Builds capacity through indirect case consultation, networking and knowledge exchange Effectively supports the identification and development of quality improvement principles

  • Ensures caregivers are provided with timely and appropriate information and referral to the Home and Community Care Support and BSO services

  • Partnering with BSO and Geriatric services to ensure appropriate use of community health professionals and reduction of service duplication

  • Participates as an integrated member supporting case conferences/meetings with clients and caregivers and community partners

Qualifications

  • A regulated healthcare professional in good standing

  • Registration with the appropriate college

  • Minimum 3 years' experience in geriatric psychiatry and/or geriatrics and/or long-term care with preference to geriatric psychiatry.

  • Training and experience with PIECES is preferred

  • Demonstrated competence in the knowledge and use of standard geriatric assessment tools and rating scales

  • Ability to enhance the local system by working with other partners in care

  • Ability to forge linkages across the community

  • Experience in identifying staff training needs

  • Experience in provision of education and training tailored to diverse organizations and settings

  • Experience in program development

  • Experience in long- term or community-based seniors care is required

  • Proficiency in the use of computer software, including MS Office Suite and virtual environments, and Internet is required

  • Fluency in English (reading, writing, speaking, comprehension).

  • Demonstrated commitment to continuing education

  • Excellent skills in adult education including negotiation, facilitation, communication, coaching and problem solving

  • Sensitivity to multi-cultural issues

  • Demonstrated ability to function independently with minimal support

  • Excellent attendance, punctuality and work record

  • Travel within the Central West LHIN, a valid drivers license and access to a car is required

  • A combination of experience and academic qualifications will be considered in selecting the preferred candidate

Additional Information

Hours: Currently Days, 8 hours, shifts including weekends and statutory holidays (subject to change in accordance with operational requirements)

Salary:
Minimum: $40.94
Maximum: $52.45
