Position Summary
In consultation and collaboration with Interventional Cardiologists, Cardiac Surgeons, Electrophysiologists, and other providers, assist in the triage of patients for interventional cardiac procedures including but not limited to coronary angiography, PCI, CTO, structural heart procedures, Pacemaker/ICD/CRT Implantation, Arrhythmia assessment and intervention and follow up care within Trillium Health Partners - Mississauga Hospital and between referring centres to facilitate timely access to services.
The triage coordinator proceeds to plan for the cardiac procedure, completes the booking, and relays information and education to the patient, family, referring physicians, referring hospital and the THP receiving units. Working with office staff, collect and enter required data into the booking grids and the CorHealth database. Acts as a resource to the program and referring centers.
Responsibilities

Works in collaboration with physicians, triage teams, physicians offices, and referring physicians and their offices, referring hospitals, anesthesia, and critical care and inpatient units.

Receives and reviews referrals from THP, regional partner hospitals, and physician offices
-
Reviews all pertinent information on referred patients.

Manages the wait list — aware of changes in status or symptoms and make changes to the bookings as indicated - Keeps Physicians informed.

Relays information to referring unit/hospital/physicians of procedure date.

Ensure the work up for procedure is completed.

Manage anticoagulation and cardiac medications pre-operative with physicians

Coordinate industry representatives for clinical support

Data entry, management and report writing of the Cardiac Care Network of Ontario WTIS-CCN Along with office staff ensure required data is collected, verified and entered into the CorHealth database application - ICD/CRT and arrhythmia management and maintenance and dept. database(s).

Outcomes data collection, entry and management of data base

Assists with quality assurance activities and prepare/enter monthly statistics

Establishing monitoring and reporting systems for quality assurance.

Coordination with other Arrhythmia Service Triage and CCN coordinators as a LHIN
Regional Centre

Participates in continuous quality improvement and evaluation
-
Knowledge of best practices for patients referred for all interventional cardiac procedures in the cardiac cath lab and CVOR
-
Performs other duties consistent with the role as required.

Support cardiac triage offices as needed.

Communicates with all team members as required

Contributes to the overall vision/mandate of the program
Key Qualifications

Current registration with CNO is required

Baccalaureate Degree in Nursing, Master's Degree Preferred

Completion of Critical Care Certificate or CCNC required
-
3 - 5 years recent Cardiac Surgery Critical Care experience required

Comprehensive knowledge of arrhythmia management and device therapy and technology required.

Clinical knowledge of 12 Lead ECG interpretation, stress test, echo, perfusion scans, coronary angiogram

Ability to think critically, effectively and prioritize patient needs

Excellent communication and interpersonal skills required
-
Strong verbal, written, interpersonal and customer service communication skills

Work collaboratively as a part of an interdisciplinary team including Regional referring centers, referring physicians and their offices, inpatient units — THP and regional, CorHealth, THP Cardiac Surgeons, Cardiologists/Electrophysiologists and the other Triage teams.

Ability to represent Trillium Health Partners and the Cardiac Health program in a positive way.
