Job Description

 

The Rehab Admissions Coordinator, considered a non-supervisory clinical

leadership position within the organization, will report to the Director, Rehabilitative & Complex Continuing Care. In this role the Rehab Admissions Coordinator will work collaboratively with the Post-Acute Program Leadership as well as other key internal/external stakeholders to develop and execute new program initiatives that align with THP Strategic Plan and Provincial Guidelines. These efforts will be aimed to facilitate quality patient outcomes while meeting the growing demand of these services through the development of standard and transparent referral principles and practices within THP and to external providers. 

 

The Rehab Admissions Coordinator will use clinical knowledge and expertise to

assess and work with hospital clinical teams to optimize the timing and accuracy of the referral process, helping to facilitate seamless transitions and hand offs between clinical providers. To this end they will serve as a content expert in rehabilitation and a central point of contact for this program of services.

This role will also play a key function in supporting external partnerships with

other Rehabilitation and Complex Continuing Care Providers through optimized relationships, referrals and patient flow processes, where Trillium Health Partners has preferred access. 

 

Key Qualifications

  • Knowledge, skill and judgement of a Registered Occupational Therapist, Registered Physiotherapist or Registered Nurse in good standing with their respective professional college in Ontario

  • Recent experience working in rehabilitation and/or complex continuing care environments

  • Working knowledge and familiarity with widely utilized clinical assessment tools (e.g. NRS, FIM, IRRS, MoCA, AlphaFIM, CMI, etc.)

  • Excellent clinical assessment skills to interpret a patient/client’s functional and medical status to determine rehab readiness and further be able to triage to the appropriate stream of recovery

  • Be able to work highly independently in fast-paced work environments

  • Excellent communication skills (both written and verbal) that demonstrate diplomacy and the mission and values of Trillium Health Partners

  • Excellent critical thinking skills to be able to recognize and proactively work through problems to achieve resolutions

  • Project management and program development skills 

  • Quality improvement experience (Data collection and interpretation to help drive innovative program improvement)

  • Experience working with internal and external leaders to achieve program deliverables
