Registered Nurse**–**West Assertive Community Treatment Team (ACTT) (Full Time Permanent)
The Role and Opportunity
The Canadian Mental Health Association is looking for a Registered Nurse (RN) for the Assertive Community Treatment Team (ACTT) in the West Office. The Assertive Community Treatment Team (ACTT) is a multidisciplinary team providing community-based support, rehabilitation and treatment for people living with mental illness. The team is based out of CMHA Toronto – Lawrence Site. Services are provided in the community and focus on stabilization, community integration, quality of life and recovery from mental illness.

Responsibilities
Reporting to the Program Manager, the Registered Nurse will work closely with the interprofessional ACT team providing nursing care, health promotion, as well as assistance with activities of daily living and support to individuals with a serious and persistent mental illness in a community setting.
The RN will provide psycho-education around meds and symptoms and assess mental status and monitor physical health needs. The role will also require the nurse to develop therapeutic alliance with clients and their support systems/significant others and work from a recovery-based perspective. To support the client the nurse will assess client needs and link to appropriate resources as well as monitor treatment regimes. The RPN/RN will support the agency’s primary health care and chronic disease management initiative.
Skills You Bring
CMHA Toronto’s ideal candidate will have an undergraduate degree in nursing and a registration and good standing with College of Nurses of Ontario as a Registered Nurse. The candidate will bring a minimum of two (2) years’ experience working with people with mental illness, experience in community mental health will be considered an asset. The candidate will have a sound mental health assessment and teaching, medication monitoring, case management, and crisis intervention. As well as have knowledge and understanding of the ACT Model and recovery-oriented practice. Experience in with addictions, forensics and dual diagnosis will be considered an asset.
The candidate will bring a solution focused approach to work and the ability to manage crisis, as well as ability to work with diverse communities and demonstrate from an anti-oppressive framework. The candidate will also have the ability to work within a multidisciplinary team as well as work independently. The candidate will also be required to have a valid Ontario Driver’s License, access to a vehicle and insurance coverage.
