About the Role:
We are seeking a compassionate and skilled Social Worker to join our interprofessional healthcare team. The Social Worker provides comprehensive psychosocial assessment, counselling, education, and advocacy to support patients and their families in navigating the impacts of illness and hospitalization. This role focuses on enhancing patient and family well-being, facilitating safe and effective discharge planning, and connecting individuals to appropriate community resources. The Social Worker also collaborates closely with healthcare providers to address the psychosocial aspects of care while contributing to quality improvement and professional development initiatives.
What you’ll be doing:
- Deliver psychosocial assessments and provide counselling, emotional support, crisis intervention, and grief support to patients and families.
- Support patients and caregivers in coping with the effects of illness and treatment, including family, financial, and social challenges.
- Facilitate discharge planning and connect patients and families with appropriate community resources and services.
- Collaborate with physicians, nurses, and other interprofessional team members to ensure coordinated, patient-centered care.
- Advocate on behalf of patients and families within the healthcare system and community.
- Provide consultation and education to healthcare teams regarding psychosocial issues and available resources.
- Maintain accurate documentation and statistics in compliance with hospital standards.
- Participate in program development, hospital committees, and quality improvement initiatives.
What you bring:
- Master’s degree in social work from an accredited university.
- Member in good standing with the Ontario College of Social Workers and Social Service Workers (OCSWSSW).
- Membership in the Ontario Association of Social Workers (OASW) is preferred.
- 3–5 years of relevant social work experience in an acute care or hospital setting.
- Experience in oncology, palliative care, or crisis intervention as an asset.
- Training or experience in grief and loss required.
- Strong knowledge of psychosocial assessment, counselling, and conflict resolution.
- Demonstrated ability to collaborate effectively with patients, families, healthcare teams, and community partners.
- Excellent organizational, problem-solving, and communication skills.
- Flexibility and ability to work under pressure in a fast-paced healthcare environment.
