Case Manager – Mental Health & Justice Prevention Program (Full-Time Permanent) The Role and Opportunity The Canadian Mental Health Association (CMHA) – Toronto is looking for a Case Manager to join our Mental Health and Justice Prevention Program . This role supports individuals living with serious mental illness who are at significant risk of criminal charges or police involvement. Working as part of a team serving East Toronto, the Case Manager will provide community-based supports, promote recovery, and foster stability through comprehensive case management and advocacy.
Salary: $54,943 – 60,203
Hours of Work: 35 per week
Closing Date: September 15, 2025

Responsibilities
- Conduct outreach and initial assessments
- Provide orientation to the program and agency
- Engage clients using a client-centered, strengths-based approach
- Develop individualized service plans and support skill-building
- Connect clients with resources, information, and advocacy
- Build and maintain trusting, respectful, and collaborative relationships
- Monitor progress, evaluate outcomes, and conduct regular follow-ups
Experience & Education Required
- Bachelor’s degree in Social Work, Social Sciences, or equivalent experience
- Minimum of 3 years of experience in community mental health and/or addictions
- Proven experience in case management
- Demonstrated ability to work with diverse and marginalized populations
- Commitment to anti-racism, anti-oppression, and 2SLGBTQIA+ affirming practices
- Proficiency in a second language is an asset
- Valid driver’s license, access to a vehicle, and appropriate insurance coverage
