About the role
Position Summary
The Couchiching Health at Home Program, Recreation Therapist builds and provides a seniors friendly recreation therapy program that includes engaging, interactive, social activities in a manner that aligns with therapeutic values of the program. Delivered to patients in their home, the Recreation Therapist works closely with the Manager and entire CHaH team, as an interdisciplinary partner in the provision of integrated, patient centered care. The CHaH Program, Recreation Therapist will utilize recreation, leisure, and play as treatment methods to support purposeful and valuable interventions that align with patients’ needs and interests.
This position works directly in the Community, with daily travel in the Couchiching Region. Mileage is paid according to OSMH policy.
PRIMARY RESPONSIBILITIES:
- Create and maintain a patient centered recreation therapy program based on best practices and stakeholder engagement, that can be provided in-person and virtually where appropriate
- Review each patient referral and care plan in order to design a patient centered plan for recreation therapy services
- Recording patients’ assessment progress and discharge status according to the institution’s policy
- Designing life skill programs, leisure activities, and special programs for therapy
- Coordinating care intervention with recreational therapy in collaboration with other healthcare professionals on the team
- Assessing reports from physiotherapists, physicians, and other professionals to determine a patient’s functioning level
- Employing music, art and dance activities and specialized therapy sessions
- Teaching clients about injury prevention, treatment and promoting overall health
- Observing and analyzing patients during treatment sessions, document and continually assess goals and progress
- Staying up-to-date with new treatments and research development
- Report findings back to care coordinator and Manager, and escalate in real-time as needed
- Supports program planning, implementation and evaluation
- Participating in a supportive and healthy working environment
- Performs other duties as may be assigned by Program Manager
Qualifications
Education:
-
Graduate of a recognized program dealing with Recreation Therapy at a diploma or degree level.
Experience:
-
Member in good standing of Therapeutic Recreation of Ontario (TRO) required
-
Minimum three (3) years’ experience in recreation therapy in adult medicine/geriatrics/eldercare preferred
-
Recent experience working within an inter-professional team preferred
-
Relevant knowledge of inpatient care environment preferred
Competencies:

Demonstrated ability to communicate effectively with others, with strong interpersonal skills
