Job Summary

Reporting to the Director, the Transition Navigator facilitates safe, effective, and patient-centered transitions across the continuum of care. The role supports discharge planning, care coordination, and access to community and transitional care services. As the primary coordinator for Stevenson@Home (hospital to home (H2H) program) and the Transitional Care Program (TCP), the Transition Navigator identifies eligible patients, coordinates transitions, and promotes continuity of care between hospital, community, and post-acute care settings.

Working collaboratively with patients, families, healthcare providers, Ontario Health@Home, and community partners, the Transition Navigator supports integrated care delivery, ALC reduction strategies, and improved patient outcomes and experience while advancing quality, patient safety, health equity, and Senior Friendly Care principles.

Qualifications And Experience

Regulated Health Professional in good standing with the applicable regulatory college (e.g., RN, SW) Current registration with the appropriate regulatory body. Minimum three (3) years recent experience in discharge planning, patient navigation, care coordination, case management, transitional care, community care, or related healthcare services. Experience working within hospital-to-home programs, transitional care programs, integrated care pathways, or community-based care models is preferred. Demonstrated knowledge of Ontario Health@Home services, Community Support Services, Long-Term Care placement processes, home and community care legislation, and healthcare system navigation. Knowledge of seniors' care, frailty, geriatric syndromes, complex chronic disease management, transitional care models, and ALC reduction strategies. Experience supporting quality improvement initiatives, program evaluation, clinical pathway implementation, and performance measurement is an asset.

Skills And Abilities

Demonstrated assessment, care coordination, critical thinking, problem-solving, and clinical decision-making skills. Excellent interpersonal, communication, facilitation, conflict resolution, and relationship-building skills. Ability to work collaboratively with patients, families, interdisciplinary teams, and external community partners. Strong organizational and time management skills with the ability to manage multiple priorities in a fast-paced environment. Demonstrated commitment to patient- and family-centred care, health equity, Senior Friendly Care, quality improvement, and patient safety. Proficiency with electronic medical records (preferably Meditech), Microsoft Office applications, virtual care technologies, and electronic documentation systems. Experience utilizing data collection, reporting, and performance measurement tools to support program monitoring and evaluation. Demonstrated understanding of privacy, confidentiality, and information management requirements. Demonstrated commitment to professional development, continuous learning, and evidence-informed practice. Hours of Work: Monday to Friday, weekend work may be required as needed.
