Your Opportunity:
The Palliative Home Care Program objective is to provide excellent palliative care to individuals & families in the community setting. Clinicians demonstrate the values inherent to Patient & Family Centered Care.
Reporting to the Care Manager of the Palliative Home Care Program, the Palliative Registered Nurse: Case Manager (CM), works as a member of an inter-professional care team. Based on the Continuing Care Case Management Competency Framework, you demonstrate ability in the areas of personal and professional accountability, systems thinking, communication and establishing collaborative relationships. The Palliative RN CM is responsible for case management and direct nursing care of palliative clients. Using a Patient- Family centered approach; you will identify client/family care needs, develop a care plan to support identified needs, coordinate and evaluate care interventions. The Palliative RN CM fosters and promotes continuity of care through collaborative relationships with the inter-professional care team, the primary care physician and other programs or services involved in the provision of client care.
PLEASE NOTE: This is an ON-SITE position that requires reporting to an office site daily. There is no opportunity for remote work.

As a Registered Nurse (RN), you will provide a wide variety of nursing services to patients, families, communities and populations, while taking necessary steps to ensure their safety and well-being. In your role, you will utilize nursing processes, through critical thinking, problem solving and decision making, as well as teach, counsel and advocate on behalf of patients and their families. You will play a leadership role in supporting an integrated and holistic approach to patient care, health promotion and maintenance. You will provide safe, quality patient and family centered care while reflecting the shared vision and values of AHS.
PLEASE NOTE: Provisional Permit Holders shall be paid $39.58 /hour - $54.16/hour. Upon being entered into the register of Registered Nurses, the rate of pay shall be adjusted to the rate of pay applicable to a Registered Nurse in accordance with AHS/UNA Article 25.02(b)(i) or (ii) as applicable.

Required Qualifications:
Completion of an accredited nursing education program.
Active or eligible for registration and practice permit with the College of Registered Nurses of Alberta (CRNA).
Current Basic Cardiac Life Support - Health Care Provider (BCLS-HCP).
If you have not practiced as a Registered Nurse within the last five (5) years, completion of the following free courses is required: CRNA Medication Management - Chapters 1 - 4 (https://connect.nurses.ab.ca/home/%2Flearning-and-development%2Flearning-modules%2F) and the NextGenU Health Assessment Resources Certification (https://courses.nextgenu.org/course/view.php?id=271#section-0)
Additional Required Qualifications:
Must have three years of community experience and/or three years of acute medical or oncology nursing experience within the past five years.
Recent Palliative Care Experience.
Strong interpersonal and communication skills.
Ability to work in an inter-professional team.
Ability to prioritize and organize many details in a timely and flexible manner.
Ability to work in a self-directed environment, independently, using sound professional judgment.
Effective decision making and critical thinking skills are required.
Proficient in the use of Microsoft Office software including Word and Outlook.
Must be physically able to perform all aspects of the position in accordance with the Hazard Identification Assessment and Control (HIAC) form. This includes the ability to lift 25lbs, walk a minimum of 3km/shift, and able to tolerate bending, lifting, twisting, in accordance with Move Safe training.
This position requires a vehicle for business use. Possession of a valid driver’s license and a motor vehicle in good repair for work related travel is required.
Orientation to this role is full-time for 6 consecutive weeks for applicants external to Home Care. Applicants must be able to commit to this 6-week requirement.
Preferred Qualifications:
One year of Palliative Home Care Experience preferable in the last two to three years.
Baccalaureate or advanced degree in Nursing.
CNA certification in Hospice Palliative Care.
Case Management Certification.
