About the role

Kemptville District Hospital is seeking an Social Worker/Discharge Navigator to join our growing team, on a part-time basis.

Qualifications

  • Bachelor’s degree in social work (BSW) or equivalent required;
  • Current certificate of registration in good standing with the Ontario College of Social Workers and Social Service Workers;
  • Relevant clinical or discharge planning experience in a healthcare setting;
  • Certification in Cognitive Behavioral Therapy, Motivational Interviewing, and Mindfulness-based stress reduction, preferred;
  • Ability to work effectively in a multi-disciplinary team environment;
  • Flexibility to adapt to organizational priorities and client needs.
  • Demonstrated comprehensive knowledge of community services, agencies, and placement facilities;
  • Demonstrated knowledge and understanding of legislation and acts applicable to patient discharge and transition (e.g. Health Care Consent Act, Mental Health Act, Public Hospital Act, Substitute Decisions Act);
  • Demonstrated knowledge of financial aid available from federal, provincial, and municipal programs;
  • Strong communication, organizational, and problem-solving skills;
  • Knowledge of computer business applications including electronic charting (EPIC);
  • Respect for individuals of diverse cultures and demonstration of culturally sensitive practice.

Starting wage:

$38.93/hr

Please note: In order to work in a healthcare facility, the Public Hospitals Act 1990, Regulation 965 requires all employees to provide proof of up-to-date immunization/vaccines for the following diseases: Tuberculin Skin Testing (TB), Measles, Mumps, Rubella (MMR), Varicella (Chicken Pox), Hepatitis B, Tetanus/Diphtheria (DTap), Influenza (during flu season) and three COVID shots. This proof must be provided to our Occupational Health Office in order to be cleared to work.
