About the role

As a member of the Community Support team the incumbent provides spiritual care and/or bereavement support to those with a life limiting illness, their care givers and those who have experienced the death of a loved one in the Halton community, in accordance with their specific program and with Acclaim Health standards, policies and procedures.

Acclaim Health announces the availability of one (1) Established Part Time Bereavement and Spiritual Care Counsellor position with the Community Support Services Department to work in the Milton/Halton Hills Community. As an Established Part Time, the individual will be required to forty-five (45) hours in a bi-weekly period. This individual will work 3 shifts per week.

Pay: $28.84 - $30.58 per hour

DUTIES & RESPONSIBLITIES:

  • Conducts initial in home or virtual client assessments to determine eligibility and to assess specific client and/or caregiver needs
  • Together with the client and/or caregiver, develops a care plan to meet identified goals
  • Provides comprehensive psychosocial, spiritual and bereavement support to clients and caregivers
  • Provides timely provision of information, system navigation and referrals to other resources as appropriate, ensuring warm transfers are made
  • Participates in the supervision, training, mentoring and support of bereavement volunteers
  • Matches volunteers with clients for bereavement support and monitors clients’ well-being while receiving support
  • Coordinates and facilitates bereavement support groups and bereavement education sessions
  • Plans and leads memorial services (virtual and/or in person) for bereavement clients and family members/friend of Acclaim Health clients who have died
  • Attends palliative rounds and participates in care conferences as needed
  • Gathers statistics for monthly and quarterly statistical reports as they relate to specific program.
  • Updates and maintains confidential client and volunteer files in accordance to established policies and procedures
  • Promotes quality/risk management, ensuring that risk/and incident management policies and procedures are followed.
  • Represents Acclaim Health in the community promoting linkages to appropriate community resources as requested by the Director.
  • Ensures that workplace conduct and activities are in accordance with the current Occupational Health and Safety Act and internal health and safety systems.
  • Performs other related duties associated with the position and as requested by Director.

QUALIFICATIONS & EDUCATION**:**

  • Undergraduate Degree in Theology, Social Work, post grad certificate in Psychotherapy, or equivalent
  • Training through CASC (Canadian Association for Spiritual Care) preferred
  • Certification in CPR-Basic Rescuer Level
  • Valid Ontario driver’s license, use of vehicle and insurance in accordance with the Acclaim Health policy

EXPERIENCE & SKILLS**:**

  • Minimum two years demonstrated experience working in spiritual care, bereavement and/or palliative care.

  • Strong understanding of hospice philosophy of care.

  • High level of comfort engaging with others in end of life discussions and bereavement experiences.

  • Understanding of mental health concerns

  • Experience working effectively with volunteers.

  • Strong verbal, written communication and interpersonal skills.

  • Knowledge and understanding of volunteerism, community resources and the Halton healthcare system

  • Strong listening, assessment and conflict resolution skills

  • Effective decision-making, problem-solving and organizational skills.

  • Demonstrated knowledge in quality/risk management.

  • Proven ability to multi-task and meet deadlines.

  • Proficient in Microsoft Office 365, AlayaCare.

  • Demonstrated ability to work independently and collaboratively as a team member. Ability to work with individuals from diverse backgrounds and experiences

  • Must be legally eligible to work in Canada
