Sinai Health is looking for an experienced Social Worker to support our Reactivation Care Centre team as part of our Allied Health- Social Work department.

About this team:

Our specialized 16 bed inpatient reactivation program is located on the beautiful and quaint UHN-RCC campus. The Sinai Health RCC program is designed for individuals who present with decreased mobility, independence and/or function as a result of an acute illness or surgery. We focus on patients with potential for functional improvement, providing tailored care plans and utilizing state-of-the-art tools to support their recovery journey. Depending on the reactivation goals patients typically experience an average length of stay of 30 to 60 days. You will collaborate with a high performing multi-disciplinary team including Registered Nurses, Registered Practical Nurses, Nurse Practitioner, OT, PT, SW, and SLP to provide high quality and safe patient care.

In this role you will:

• Provide a range of social work services including assessment, crisis intervention, counselling; comprehensive discharge planning with individuals and families.
• Conduct psychosocial assessments and provide interventions based on a variety of issues such as aging and dementia, homelessness, addictions, child protection, grief counselling, mental health and domestic violence
• Link with community agencies to ensure coordination of appropriate services for patients.

Job Requirements

Job Requirements:

Mandatory

• Master's Degree in Social Work

• Current registration with the Ontario College of Social Workers and Social Service Workers

Preferred

• Experience with a range of care in a rehab setting

• Experience with consent and capacity

Skills and Knowledge

• Demonstrated ability in individual and family counselling

• Knowledge of community supports and resources

• Demonstrated ability to work independently and in a team environment

• Excellent interpersonal, verbal and written communication skills

• Demonstrated interest in education and research

• Demonstrated satisfactory work performance and attendance record
