About the Dufferin HART Hub
The Dufferin HART Hub is dedicated to improving outcomes for individuals in Dufferin County experiencing homelessness, mental health challenges, and substance use issues. Role Overview
We are seeking a dynamic and compassionate Mental Health Workerto be an integral part of the HART Hub Dufferin collaborating with a multidisciplinary team delivering comprehensive, integrated care services to vulnerable individuals. The Mental Health Worker will provide brief mental health case management support and deliver time-limited, client-centered interventions. They will use evidence-based approaches to monitor these interventions and work closely with a variety of professionals.
Reporting Function:
The Mental Health Worker will report directly to the HHD Manager. They will work with both internal and external partners and will interact with all levels within the organization.
Duties and Responsibilities:
- Collaborate in developing and implementing individualized wellness plans with clients
- Use standardized assessment tools designed to improve and/or stabilize client’s physical and mental health
- Offer a strength-based approach that recognizes client’s unique needs
- Assist with activities of daily living, conflict resolution, and crisis avoidance
- Provide comprehensive, holistic, and person/family centered access to services that address the social determinants of health
- Partner with clients and their families as members of the health care team
- Develop and maintain a community resource network
- Refer to partnering organizations and provide linkages to community supports
- Work independently and collaboratively within a multidisciplinary team
- Understand and be able to integrate a philosophy of person and family centered care into daily responsibilities
- Demonstrate a strong commitment to quality care and service delivery based on the HART Hub Mission and Values
- Ensure appropriate records are kept and ensure that statistical information is maintained as per agency standards
- Adhere to agency Health & Safety Program and the Client Safety Framework in the promotion of client safety
- Perform additional duties as required/delegated by Management
Qualifications:
- Post-secondary education in Social Work, Health Services, Psychology or equivalent
- 3 years providing Mental Health case management or equivalent care
- Demonstrated knowledge of serious mental illness, concurrent disorders, and/or dual diagnosis, and experience with assessment
- Experience working within the homelessness sector
- Knowledge of evidence-based practice for mental health services
- Excellent assessment, risk management, and crisis intervention skills
- Expertise in collaboration with healthcare services
- Knowledge of MS Suite (Word, Excel, Power Point and Outlook)
- A vehicle, valid driver’s license, and adequate insurance coverage
- Valid Certification in First Aid & CPR
- Training in ASIST, CPI and Naloxone administration
Working Conditions and Requirements:
- Day and evening with occasional weekend shifts
- Able to sit/stand for prolonged periods of time (building inspections, site meetings, unit inspections, etc.)
- Computer screen use (multiple screens if applicable) for prolonged periods of time
- Able to lift up to 20lbs (product, devices etc.)
- Vehicle with adequate insurance coverage to travel to community sites is required
