About the role
Summary of Duties:
The Clinical Dietitian provides nutrition management for patients living with Chronic Kidney Disease throughout the continuum of care in accordance with Clinical Nutrition care standards as well as the Patient- Focused Care standards of Sunnybrook Health Sciences Centre. The candidate will be responsible for providing exceptional nutrition patient care as a part of Sunnybrook’s Outpatient Nephrology Program (Multi-care Kidney Clinic, Hemodialysis). The successful candidate will participate in the advancement of patient care through attending interprofessional rounds, participating in unit-based and organizational committees, and engaging in quality initiatives and practice-based research. The candidate will be expected to be an active member of the clinical nutrition team and participate in updating nutrition resources used in the area. The candidate may also participate in Sunnybrook’s dietetic internship program providing education and training to Dietetic interns. The candidate may be expected to participate as a preceptor or in another supportive role.
Qualifications/Skills:
- 1-2 years’ experience with the nephrology population preferred;
- 1-2 years’ experience educating patients with an understanding of principles of adult education;
- Current registration in good standing with College of Dietitians of Ontario mandatory;
- Excellent communication and organizational skills;
- Strong team building and conflict resolution skills;
- Able to work independently as well as co-operatively with peers and multidisciplinary team members; adapting to a changing environment and responding with initiative
- Able to practice with a Patient-Focused Care approach, with an understanding and appreciation of the nephrology population; successful ability to practice with cultural competence an asset
