Position Summary : The successful applicant will provide leadership to the Peritoneal Dialysis Unit in collaboration with the Clinical Manager. The role includes the following and not limited to: plan, coordinate and direct patient care activities; act as a resource, mentor and role model for staff members; adapt to changing workloads and competing priorities; actively participate in unit initiatives and committees. The Peritoneal Dialysis Unit is engaged in caring for patients who have selected Peritoneal Dialysis as a renal replacement therapy. The successful applicant will work in collaboration with the Nephrologists, the Allied Health team and Nursing team to support the coordination of education, training and monitoring of patients. The successful candidate will incorporate the attitude, values, knowledge and skills required to support clinical needs and independence; introducing the concept of self-management, self-care and promoting a culture of “Home First”.

Position Requirements:

  • Current Certificate of Competence – College of Nurses of Ontario in good standing, BSCN preferred.
  • Canadian Nursing Association (CNA) Nephrology Nursing Certification required; if not already obtained, proof of CNA Nephrology Certification enrollment is required at the time of application and proof of successful completion is required within 3 months of hire/transfer date
  • Minimum of 5 years current renal experience with a minimum of 3 years Peritoneal Dialysis experience required.
  • Skills in assessing, planning, implementing and evaluating the ongoing clinical management of dialysis therapy.
  • Demonstrated knowledge of nephrology theory and clinical renal nursing practice.
  • Resource nurse experience is preferred.
  • Demonstrated leadership skills: Prioritizing, problem solving and excellent critical thinking skills.
  • Evidence of effective change management skills
  • BCLS
  • Demonstrated experience, ability and knowledge in the principles of adult teaching and patient teaching skills.
  • Demonstrated ability to work autonomously and within team environment.
  • Demonstrated good interpersonal and communication skills with an ability to work with a multidisciplinary team, including other health team members, patients, family members and visitors.
  • Ability to be flexible and effectively work in a busy environment with changing priorities and deadlines.
  • Demonstrated sound judgement, good observation and patient assessment skills, tack and empathy.
  • Demonstrated expertise in management of health problems and support needs for peritoneal dialysis patients in the home setting.
  • Computer skills and nephrocare/Cerner experience is required.
  • Demonstrated understanding of and commitment to Grand River Hospital’s comprehensive safety programs and practices including staff, patient and environmental safety, and participation in regular in-service education in this area.
  • Must have own reliable vehicle and a valid driver’s license for home visits and on-call.
  • Demonstrated good attendance record
