We're seeking an experienced Care Planner & Relationship Coordinator to join our team.
Compensation Package:
- Base Salary: $36,000 - $40,000 annually
- Additional Earning Potential: Up to $8,000-$12,000 annually through:
- Client retention incentives
- Quarterly performance rewards
Location: Serving Hamilton, Oakville, Burlington, and Mississauga.
Schedule: Full-time, Monday to Friday (occasional evening/weekend flexibility required)
Position Overview: We seek a dynamic healthcare professional to develop business relationships, coordinate client services, and ensure quality care through PSWs with consistent supervision and training. This role involves significant fieldwork, spending 50-60% of the time across our service area.
Key Responsibilities:
The Care Planner & Relationship Coordinator will drive business growth through active networking and relationship-building with healthcare partners, including hospitals, clinics, retirement homes, and community organizations. This includes conducting regular networking visits, participating in healthcare forums, and representing our company at community events. The role involves building and maintaining relationships with healthcare providers and community partners who can refer clients to our service. This includes managing the entire process from receiving a referral to converting it into an active client - from initial contact to care plan implementation. The coordinator will track all leads, conduct assessments, present care solutions, and ensure a smooth transition into our service.
A key aspect of this position involves maintaining our high standards of care through comprehensive quality assurance. The coordinator will conduct initial client assessments, develop personalized care plans, and perform regular quality assurance visits to client homes. This includes monitoring service delivery, evaluating PSW performance, and ensuring client satisfaction through regular feedback collection and prompt resolution of concerns. Detailed documentation of all visits, assessments, and interactions is essential to maintain service excellence.
The position also encompasses staff management and development responsibilities. The coordinator will participate in PSW recruitment and screening processes, conduct orientation sessions for new hires, and implement ongoing training programs. They will provide continuous education and support to our PSW team, monitor their performance, and ensure compliance with healthcare standards and best practices. This role is crucial in maintaining our reputation for excellence in home healthcare services through proper staff development and quality control measures.
Success in this role requires extensive fieldwork throughout the Hamilton, Burlington, Oakville, and Mississauga regions, focusing on building and maintaining strong community relationships while ensuring the highest standards of care delivery.
Required Qualifications:
Must have a PSW Certificate
Fluent in English (written and verbal); other languages an asset
A healthcare-related degree or diploma is an asset
Minimum 5 years of hands-on PSW experience, including 1-3 years in supervision/coordination
Valid G driver's license with clean driving record
Reliable vehicle with proper insurance
Excellent communication and interpersonal skills
Proficiency in healthcare documentation and Microsoft Office (Word, Excel)
Scheduling software experience is an asset
Criminal Background Check and Vulnerable Sector Screening required
Essential Skills & Qualities:
Excellent time management and organizational abilities
Strong networking and relationship-building capabilities
Professional presence and presentation skills
Ability to work independently and manage multiple priorities
Comfortable with extensive daily travel throughout the service area
Detail-oriented with strong documentation skills
