St. Michael's Hospital Mental Health and Addiction Service (MHAS) provides care to individuals with serious mental illness, addictions and complex medical needs. Ambulatory Mental Health Service offers outpatient service through Urgent Care Program, Mental Health Assessment Clinic, and Mental Health Recovery Program, serving patients at different acuity and chronicity levels. The primary role of the Social Worker is to provide individual (1:1) clinical work in these programs to support timely access and flow. The role will also be responsible for facilitating and coordinating evidence-based group programing in the ambulatory setting. This role will involve building capacity in group therapies, providing education to students and supervision to other facilitators.
Counseling, discharge planning, resource referral, consultation and collaboration, teaching, research/quality improvement and patient and family education are required elements of this position.
DUTIES & RESPONSIBILITIES:

Provide case management by initiating and maintaining primary responsibility for developing a complex discharge plan in collaboration with Ambulatory Mental Health Programs
-
Provide brief evidence based psychotherapies and supportive counseling to meet complex client needs while supporting timely access to care. Preference will be given to those with experience providing trauma informed therapy, CBT and DBT.

Co-ordinate and facilitate family meetings and case conferences
-
Facilitate and coordinate evidence-based group therapy program patients in the Ambulatory Mental Health Programs

Coordinate intake and assessment for group therapy program
-
Work with the interdisciplinary team to develop and presents proposals for group therapy interventions to meet the needs of patient/families, community and staff
-
Maintain all required administrative documentation, such as workload statistics, patient rosters, equipment lists, etc.

Follow and keep up to date on hospital and department policies and procedures
-
Is committed to, and participates in, ongoing quality improvement in the organization
-
Promote professional governance and effective hospital operations through, for example, participation in professional practice councils, unit/departmental meetings and hospital committees
QUALIFICATIONS:
-
Master's Degree in a health related health discipline required and registered with relevant regulatory college in good standing i.e. Ontario College of Social Workers and Social Service Workers (OCSWSSW), College of Occupational Therapists of Ontario (COTO), etc.

Current CPR certification
-
Minimum 3 - 5 years direct experience in acute hospital and/or outpatient mental health setting.

Completion of post-graduate counselling certificate programs preferred
-
Solid experience in providing brief structured psychotherapies and supportive counseling to a range of populations. Preference will be given to those with experience providing CBT and DBT
-
Experience in trauma informed care preferred

Experience in organizing and facilitating psychotherapeutic groups; preference given to those with experience running DBT/CBT/Mindfulness groups
-
Familiarity with quality improvement science an asset

Excellent communication, critical thinking and interpersonal skills
-
Proven flexibility and adaptability in a fast-paced environment

Excellent time management, organizational and prioritization skills as relates to patient care
· Actively contributes to the academic mandate of the organization as an educator, resource or support for staff, students and members of the academic and professional community
