Job Summary

You will be an integral member of the Services for Seniors Shared Services Team, responsible for working with the Region’s three Long-Term Care (LTC) homes to ensure adherence to established clinical practice, support compliance with legislated and regulated standards, and develop new clinical best practices for resident care. You will work closely with Medical Directors, physicians and clinical partners providing strategic leadership for professional practice ensuring excellence in nursing and resident centered care.

You will report to the Manager, IPAC and Compliance, and work closely with the Senior Nursing Managers of the LTC homes and the Shared Services team to identify, manage, implement and evaluate clinical and IPAC programs, policies and procedures, skills assessments, and training programs to meet identified needs.

You will lead professional practice and hold an integral role in developing and maintaining structures that support a quality professional practice environment.

Duties & Responsibilities

  • Provide leadership and expert consultative advice on nursing issues including scope of practice, resource planning and clinical informatics to enhance the practice environment and achieve quality care.
  • Support implementation of IPAC practices that comply with statutory, regulatory and other requirements of the home and involvement with local Public Health and IPAC Community of Practice and/or IPAC Hub.
  • Participate in research and promote the utilization of appropriate research to ensure best practice in clinical care.
  • Share responsibility with other members of the Leadership team for the development and ongoing monitoring & improvement of required programs under the Fixing Long Term Care Act, 2021.
  • Work with the Quality Improvement team to ensure the completion of the quality protocols and program evaluations. Creates action plans and ensures follow-up actions are completed.
  • Collect and interpret data to develop strategies and ensure appropriate protocols are implemented to respond to a variety of quality indicators (i.e. responsive behaviours, falls, medication, infections, skin breakdown, behaviours, tube feeding etc. using validated quality improvement methodologies and tools.
  • Collaborate with the Manager, Risk and Quality Improvement to conduct audits and investigations on high-risk care issues and MLTC inspection data, evaluate current processes and lead collaborative teams in process improvements and program development, providing coaching and recommendations for corrective action.
  • Build strategic networks, alliances and partnerships with various internal and external partners including Behavioural Supports Ontario, regulatory bodies, care partners and service providers to cultivate “evidence informed practice”
  • Support clinical practice through regular meetings, orientation, mentorship, consultation and guidance on complex situations.
  • Develop educational strategies to support clinical excellence in the LTC homes.
  • Support and participate in nursing recruitment initiatives to support retention of skilled nurses.
  • Develop, implement and lead various clinical practice initiatives and committees including the Professional Advisory Committee.
  • Perform other duties as assigned which are directly related to the responsibilities of the position.

Skills & Qualifications

  • Bachelor of Science in Nursing and is a Registered Nurse with the College of Nurses of Ontario.
  • A minimum of five (5) years’ experience demonstrating progressive leadership with the ability to provide clinical direction and professional guidance on standards of practice to the nursing team in Long Term Care.
  • Must have knowledge and experience in infection prevention and control. Certification through Infection Prevention and Control Canada (IPAC) would be an asset.
  • Comprehensive knowledge of the Fixing Long Term Care Act, 2021 and Regulations 246/22, Inspection Protocols and other applicable legislation is required.
  • A demonstrated understanding of quality improvement methodologies and tools with a proven ability to analyze data, implement quality improvement initiatives, solve problems and resolve conflicts is required.
  • Must have experience leading large-scale projects and best practice initiatives and be able to effectively, motivate, lead and train in a fast-paced environment.
  • Must communicate effectively both orally and in writing with all levels of the organization and have well-developed report writing and presentation skills.
  • Self-directed, result orientated and possess strong time management and project management skills.

  • Travel will be required,incumbent must provide their own transportation. The incumbent will be required to provide a copy of their driver’s license by their first day of employment.
