Position Summary:
Trillium Health Partners (THP) is excited to hire for our 26 bed Geriatric Unit at our Mississauga Hospital location and is recruiting an individual who is highly skilled, agile and adaptable with advanced critical thinking and inter-professional skills. As a member of the Geriatric Medicine Inter-professional team you will have the opportunity to provide patient-centred care to the frail older adult with the goal to prevent cognitive and functional decline during admission and participate in plans for same discharge home.
This role will report to the Clinical Manager of Seniors' Services.
Job Description:
- Assessment of patients with communicative, swallowing and cognitive-communication dysfunction of the cardio-pulmonary, musculo-skeletal and neurological systems and co-creation of goals with the patient and family
- Planning and implementation of treatment plans to improve communication, swallowing ability and overall participation in life activities
- Participate in patient discharge planning
- Document patient assessment and treatment in the health record
- Collaboration with the inter-professional healthcare team, patients and their families to achieve patient goals and augment the treatment plan as needed
- Providing education to the inter-professional team to support evidence based care
- Assigning to and supervision of support personnel
- Involvement in clinical education of various professions, including preceptorship of SLP students
Qualifications:
- Masters degree in Speech-Language Pathology from an accredited institution.
- Licensed with the College of Audiologists & Speech-Language Pathologists of Ontario (CASLPO)
- Previous experience with assessment and management of communication and swallowing disorders in a hospital setting, including video- fluoroscopic assessment
- MBSImp certification is preferred or willingness to be certified
- Demonstrated commitment to ongoing professional development and student learning
- Strong case management skills and demonstrated ability to work in a fast paced team environment
- Superior interpersonal communication skills, including verbal and written communication
- Interest in potential professional and career development opportunities
- Satisfactory attendance and performance required
- Experience with acute, rehab and chronic adult communication and swallowing disorders in a hospital setting
- Experience with complex geriatric patients with co-morbid neurological, surgical and medical conditions
- Experience with end of life care is preferred Experience with tracheostomies is preferred
