St. Joseph’s Continuing Care Centre is seeking a Temporary Full-Time Registered Dietitian. We offer a competitive salary range between $39.04 - $49.75, + 12% in lieu of benefits, 7.5 hour shifts, HOOPP Pension plan and access to our Employee Assistance Program (EAP).

Descriptions:

  • Provides individualized nutrition and hydration assessment, nutrition risk identification, care planning, monitoring, evaluation, and reassessment for all residents/rehabilitation patients as requested;

  • Assesses nutrition status / completion of Clinical Assessment Protocols (CAPS) according to policy for all residents/patients on admission, and annually according to the RAI-MDS process and timelines;

  • Assesses nutrition status, completion of CAPS when there is a significant change in status for residents/patients at high nutrition risk, and those at moderatenutrition risk who are considered by the Registered Dietitian (RD) to require ongoing RD monitoring;

  • Identifies residents/patients for whom follow-up can be appropriately delegated to the Food Service Supervisor;

  • Ensures that meals, snacks and beverages are provided to support residents’/patients’ optimal nutrition status, and manages nutrition and hydration issues including disease processes, healthy weights, laxation, and skin integrity;

  • Plans and orders nutritional supplements according to residents’/patients’ assessed needs;

QUALIFICATIONS:

  • Graduate of a recognized University with a Bachelor of Science Degree in Dietetics ( or equivalent)

  • Completion of a recognized Internship Program

  • Membership in Good Standing with the College of Dietitians of Ontario is required.
