Job Description:
POSITION SUMMARY: Under the general supervision and direction of the Director of Nursing, provides varied adapted and individualized restorative programs, which assist residents in achieving their emotional, physical, and social potential.
MAJOR RESPONSIBILITIES:
- Conducts rehabilitative exercise programs as referred by nurse, physiotherapist, occupational therapist, or speech therapist on doctor's orders.
- Assesses, develops, initiates, and maintains opportunities for resident activation and re-motivation based on residents' interest and level of capability.
- Establishes initial socialization/re-motivation opportunities for residents in need of help and utilizes recreational means to do so.
- Maintains residents' progress notes and documents on charts as part of the multidisciplinary health care team.
- Participates in resident conferences.
- Assesses, issues, and monitors the use of therapeutic aids and devices.
- Liaises with other sections, health care professionals, residents, families, students, and volunteers.
- Attends meetings and educational programs.
- Ensures availability of restorative care programming supplies and ensures equipment is in good working order.
- Performs such other related duties as may be assigned.
QUALIFICATIONS:
Education/Experience:
- Graduate of an approved rehabilitation assistant, restorative care, adjuvant, or kinesiology program or a regulated nursing program with restorative care background and experience
Skills/Abilities:
- A genuine interest and understanding of the needs of the frail elderly is necessary
- Ability to communicate effectively and concisely, both orally and in writing
- Must be competent within the meaning of the Ontario Occupational Health and Safety Act
CONDITIONS OF EMPLOYMENT:
- Must maintain current first aid certification
- Must provide proof of full COVID-19 vaccination at point of conditional offer
- Prior to commencing employment, will be required to review and attest/affirm to all Pioneer Ridge Staff Immunization Policies
- Must undergo a successful Police Vulnerable Sector Check (PVSC)
- Must have successfully undergone a "two step" tuberculosis skin test within the 12 months prior to employment
- Prior to commencing employment, must undergo a successful Offence & Professional Misconduct Declaration Process
- Must successfully complete all mandatory training as required by the Corporation on an annual basis
