Position Summary
The Social Worker provides comprehensive assessment, care planning and treatment coordination for designated clients and their families/ support systems where the social, economic, emotional or psychological circumstances presents barriers to care and or optimum level of functioning. Working collaboratively with members of the patient flow team, other health care providers and the inter-disciplinary team, the Social Worker also provides care coordination, system navigation and advocacy, education and linkage to relevant specialized complex care resources while facilitating patient self-management.
The HISH social worker will focus on outpatients. Home visits will be required. Travel will be required.
Qualifications
Education:
-
BSW minimal qualification, MSW preferred
-
Current full registration/membership in the Ontario College of Social Workers and Social Service Workers (OCSWSSW) required
Experience:
-
Two (2) years’ experience working with an inter-disciplinary team in a health care setting required
-
Two (2) to five (5) years’ experience within an acute care hospital setting preferred
-
Two (2) to five (5) years’ experience within a home care setting preferred
Competencies:
- Experience providing scope of practice social work to patients in acute care, chronic care, home care
