Right now we are looking for a Registered Dietitian to work with our Nephrology Program. **Employment Status: Permanent Full-time
Hours of Work:** Available to work Monday to Friday, full days with flexibility to meet patient needs. Location: Humber River Health, Church (Suject to change) Labour Group: OPSEU 590 Reporting Relationship: Manager, Dialysis (Church) Hourly Rate: $41.68 - $53.28
Responsibilities
- As a member of the health care team, this position assumes the responsibility for assessment, planning, implementation, care coordination and evaluation of patients who referred for diabetes care and management.
- Contributes to the development of evidence based professional practice model of care.
- Flexibility in caseload and program assignment.
- Able to manage patient caseload in a busy acute care setting.
- Clinical supervision of clinical nutrition students/interns.
Qualifications
- Bachelor’s degree in Food and Nutrition
- M.Sc. in Nutrition or related field preferred
- Completion of an accredited dietetic internship program and a member (or eligibility to become a member) of the Dietitians of Canada.
- Must be a member in good standing with the College of Dietitians of Ontario.
- Certification or evidence of pursuing certification as a Diabetes Educator.
- Excellent knowledge of clinical nutrition principles and trends.
- Experience in applying principles of adult education in ambulatory care and the ability to teach in an individual or group setting.
- Experience in program development and evaluation.
- Knowledge of community resources and experience in community outreach preferred.
- Recent experience with Nephrology/Dialysis and chronic illness is preferred.
- Minimum three ( 3) years’ experience in diabetes care and management.
- Excellent organizational skills, with an ability to set priorities and work effectively in a fast paced environment.
- Demonstrated excellent written and verbal communication skills, including an ability to encourage, support and motivate patients and/or their families from a multi-cultural community
- Current knowledge and sensitivity to patient multicultural needs.
- Demonstrated initiative in improving work skills and processes.
- Computer skills required; ability to manage electronic recording, workload and meet submission timelines.
- Demonstrated ability to work independently and as a member of the healthcare team.
- Demonstrates Patient, Family Centred care approaches and engages in reflective practice
- Excellent communication skills (oral and written) facilitating effective communication with patients, peers, other healthcare team members and volunteers.
- Demonstrated ability to conduct research in order to develop resources/services and improve policies affecting the program.
- Excellent interpersonal skills with an ability to cultivate and maintain mutually supportive working relationships with other members of the health care team
- Excellent decision making skills.
- Ability to speak a second language an asset.
- Must be proficient in Microsoft Word, Access, Excel, etc.
- Excellent attendance and discipline free record required.
