Pay Range

Base Salary: $92,000.00 - $116,000.00 (commensurate with experience)

Reporting to the Director, Health System Solutions & Partnerships, you will provide leadership and oversight for programs related to integrated service delivery systems.

This role is responsible for building, developing, implementing, and evaluating emerging partnerships and programs, including the delivery of high-quality home care services. These services may involve multidisciplinary services and/or community support services through VHA’s Responsive Community Care team.

You will play a key role in the internal adoption of external initiatives related to new hospital delivery models and oversee the daily operations of hospital programs and projects. Collaboration is essential in this role, as you will work closely with various VHA departments to execute new hospital and Ontario Health Team (OHT) care models, while coordinating with health system partners such as hospitals, primary care, Ontario Health Teams, municipal governments, home care organizations, and community support services to ensure seamless, integrated care for clients. #LI-CF1 #INVHACorp

Key Areas of Accountability:

VHA’s Manager, Health System Solutions & Partnerships is accountable for the following responsibilities, but not limited to:

  • Manage and oversee the day-to-day operational performance of hospital & OHT partnership programs to ensure key performance indicators and service delivery standards are met
  • Accountable for quality oversight, ensuring professional practice standards are upheld contributing to client safety and overall client experience
  • Manage program financial performance including budget oversight and management of resource/staffing utilization
  • Assist with the management of workforce planning and human resources assigned to the program – including clinical, administrative, scheduling, field and systems supervision and health and safety.
  • Build and maintain positive business relationships with funders and health system partners and effectively collaborate to support new and existing programs and services.
  • Lead, coach and support a leadership team, fostering a culture of accountability, collaboration, continuous quality improvement

Required Qualifications:

  • Graduate of a recognized university program (Masters preferred).
  • A regulated healthcare professional in good standing with the professional college.
  • Advanced knowledge of service delivery and operations in the Health Care sector, preferably in home & community care sector with 5+ years of progressive leadership experience.
  • Experience in building and managing relationships and partnerships related to health system integration.
  • Experience working with integrated and/or multidisciplinary programs in the home & community care.
  • Extensive knowledge of home and community care programs and resources.
  • Client-centered and customer focused.
  • Excellent program planning and project management skills.
  • Strong team building and staff development capabilities.
  • Motivated self-starter who works well both independently and with a team to drive results.
  • Demonstrated skills in financial management, human resource management and community liaison.
  • Knowledge of Continuous Quality Improvement processes.
  • Skills and experience in customer service techniques such as de-escalation and dispute resolution.
  • Demonstrated ability to lead program development and implementation.
  • Excellent interpersonal, verbal communication skills and superior writing skills.
  • Displays a professional, positive attitude and maintains enthusiasm
  • Excellent organizational skills and attention to both detail and the “big picture”
  • Flexible; ability to handle the changing health care environment
  • Ability to make complex decisions with timely follow-up
  • Good computer skills and technology knowledge including word processing, ppt and spreadsheet applications.
  • Demonstrated interest in professional self-development and maintenance of membership in the appropriate professional association(s) and respective College.
  • Flexibility to travel occasionally.
  • Availability to participate in VHA’s On-Call roster.
