DESCRIPTION:

As part of the Clinical Support Team at KidsInclusive, the System Navigator will provide support to families of children and youth with multiple and/or complex special needs by acting as the initial point of contact at KidsInclusive as well as reviewing and facilitating referrals to other services. The System Navigator collaborates with internal staff, referral sources and community agencies to determine appropriate services. The System Navigator further assists families who are moving between service agencies and supports the ‘every door is the right door’ approach to service provision. The System Navigator assists in providing departmental support; documents his/her involvement in client care; maintains communication with all other staff members; supports departmental efforts in Continuous Quality Improvement (CQI) activities; participates in education orientation and public relations, as required; maintains a safe work environment; performs other related duties as assigned.

DUTIES AND RESPONSIBILITIES:

Clinical:

  • Reviews all referrals received by KidsInclusive; including verification of all clinical and demographic information as required.

  • Determines appropriate service pathway at KidsInclusive by consulting with team leaders.

  • Obtain consent to forward the referral information to the appropriate Service Provider Organization or other agencies as required.

  • Obtains further information as required from referral source, community agencies (with consent).

  • Be knowledgeable about and provide information to families and referring sources of appropriate services in the community when appropriate.

  • Provides written documentation to referring source and family if KidsInclusive is not able to provide the service being requested along with information of appropriate service / resources in the community.

  • Provide information about KidsInclusive to families, referral sources and community partners.

  • Monitor the status of referrals as they move through the process, identifies any issues or unusual delays and alert leaders as required.

  • Assists with the transition of services to/from other agencies who provide similar services.

Administrative:

  • Maintain a record of all daily activities in AlayaCare as per KidsInclusive policy.

  • Ensure maintenance of all client information, in accordance with privacy legislation and KHSC/KidsInclusive policies.

  • Ensures client information and demographic data are properly entered into the client record.

  • Identify and document important ‘intake notes’ including alerts.

  • Participate in team meetings, discipline meetings and KidsInclusive meetings as required.

  • Provide mentoring/ student placement and shadowing opportunities as requested by management.

  • Provide input to the development of department and or hospital quality improvement procedures as requested.

  • Facilitate working relationships with providers in the children’s services, health and educations sectors, in order to obtain and share relevant information regarding services.

  • Discharge/ admit child to appropriate programs on AlayaCare as per guidelines provided.

  • Complies with relevant statistical, clinical and administrative reporting requirements.

  • Complies with KHSC and KidsInclusive policies and procedures.

  • Carries out duties in a way that embodies the mission and values of KidsInclusive and HDH and works co-operatively with fellow employees, supervisors and departments toward the achievement of the organization’s overall goals and objectives.

  • Assumes other duties that are assigned in accordance with experience, training and qualifications to which legislated requirements may be applicable.

Educational:

  • Maintain a standard of professional education by attending educational programs and workshops; transmit information obtained from these workshops to fellow employees.

  • Complete e-learning modules as required by KHSC/KidsInclusive policies.

  • Assume educational responsibilities as required by KidsInclusive.

  • Maintain a record of educational activities on AlayaCare.

MINIMUM QUALIFICATIONS:

  • Degree or diploma in areas of child/youth/family studies/ social services or equivalent combination of education and experience.

  • An understanding of the work of Children’s Treatment Centres in Ontario and the broader network of services for children and youth in our community.

  • Experience working in children’s services sector, preferably with children and youth with special needs and their families.

  • Ability to understand and interpret client’s development need(s) as indicated in referral package.

  • Excellent communication and interpersonal skills; ability to deal with a wide variety of personalities in a calm and professional manner.

  • Good organizational skills; ability to multi-task and work in a busy, high paced environment.

  • Current BCLS at the Health Care Provider level.

  • Current Criminal Reference Check (CRC) that includes a Vulnerable Sector Search administered by the Canadian Police Information Centre (CPIC).

  • Proven good attendance record.

  • Access to a reliable personal vehicle along with a valid driver’s license and automobile insurance as per KHSC policy.

  • Proficiency in speaking and comprehending both English and French is an asset.

  • Experience working in a Children’s Treatment Centre is an asset.

  • Proven ability to attend work regularly.

  • Satisfactory criminal reference check and vulnerable sector search required.

PHYSICAL REQUIREMENTS:

The incumbent will be required to meet the physical demands as described for the area of work.
