Position Summary:

Reporting to the Program Manager at Insight Health Solutions, the Nurse Practitioner will utilize their comprehensive theoretical knowledge base and advanced level of clinical competence to support the development and clinical needs of exciting innovative programs at Insight Health Solutions.

This role will primarily include supporting the development, implementation and evaluation of Infusion Clinic aimed at enhancing the efficient delivery of intravenous iron (IVFe), particularly Monoferric to Iron Deficient Anemic (IDA) mothers postpartum initially. Based on the results of this initiative, there may also be the prospect of expanding this program to other populations in the future.

This Nurse Practitioner will also provide coverage as required in THP Solutions Clinic, a virtual care platform that delivers virtual care to and management for episodic issues. In addition, the NP will provide coverage for the Seamless Care Optimizing Patient Experience (SCOPE) program to support navigation of hospital and community services for primary care providers in the community to reduce avoidable ED utilization.

The NP will also provide in-person medical assessment and treatment services to employers, and patients directly. Furthermore, the Nurse Practitioner will support IHS' Occupational Health services for employers, businesses and other clients, including pre-employment occupational health assessments, fitness-to-duty examinations, point-of-care testing (POCT), laboratory specimen collection, immunizations, general physical examinations, and other occupational health medical assessments, as required.

This Nurse Practitioner will act as a clinical resource and lead the clinical design, implementation, and operational support for upcoming clinical initiatives requiring Nurse Practitioner expertise, such as but not limited to, a new Pain Clinic, providing clinical expertise for development of clinical protocols, patient flow processes, and monitoring quality indicators to ensure safe, effective, and patient-centered care.

The Nurse Practitioner (NP) will utilize a comprehensive theoretical knowledge base and an advanced level of clinical competence in caring for patients within these programs. This will involve performing advanced patient care assessments and coordinating care for patients with complex co-morbidities, ordering appropriate diagnostic tests, pain and symptom management, and patient education. The NP will be instrumental in helping patients navigate the system to improve access and optimize safe and high quality outcomes thereby, enhancing a positive patient experience closer to home.

As a nursing leader, the Nurse Practitioner will perform quality improvement, monitoring and evaluation, complete administrative activities and participate in the development/revisions of policy and procedure. Other key components of the role may entail supporting research activities; developing, delivering, coordinating and participating in education and professional development activities/programs; performing activities to further develop and strengthen the profession; acting as a resource and mentor; and serving as a consultant to individuals and groups within the professional community and other hospitals/agencies.

Responsibilities: ( The NP works within 4 domains of practice)

Clinical Domain:

  • Demonstrates the NP competencies in providing clinical care to Iron Deficient Anemic (IDA) mothers postpartum. This patient population may also grow in the future.
  • Administer IV iron infusions following established protocols, ensuring correct dosage, dilution, and infusion rates. Proficiency in starting IVs is key.
  • Monitor patients during and after infusions for adverse reactions and manage accordingly.
  • The NP works autonomously to diagnose order and interpret diagnostic tests, prescribe pharmaceuticals.
  • Leads the interprofessional team to provide clinical care for, including making independent decisions for patient treatment and discharge, including completion of discharge summaries and transfer of accountability to community care providers.
  • Supports the operational readiness of the clinic by monitoring supply levels and identifying when replenishment may be required. Collaborates with clinic staff to ensure appropriate inventory is maintained to support safe and efficient patient care.
  • Ensure infusion clinic readiness for emergency management of infusion reactions, including anaphylaxis.
  • Maintain up-to-date knowledge of IV iron formulations, safety protocols, and emerging best practices.
  • Provide support for the clinical design, implementation, and operational support for any upcoming IHS clinical initiatives, including assessing patients, developing individualized care plans, coordinating multidisciplinary input, and supporting the day-to-day operations to ensure clinic readiness and smooth workflow.
  • Conduct occupational health assessments, including fitness-to-work evaluations, POCT, immunizations, and physical examinations for various employer clients.
  • Provide clinical care both in virtual and in-person capacity.
  • Participate in quality improvement initiatives and contribute to protocol development and refinement.
  • Conduct medical assessment and follow up.
  • Work collaboratively with the interdisciplinary team and arrange follow up care as required.
  • Liaise with family physicians, community nurse practitioners and other medical specialists to connect patients to community resources.
  • Refer for diagnostic imaging, laboratory tests and other specialist care as required. Document clinical data, assessment findings, diagnoses, plans of care, interventions, patient responses, and clinical rationale in a timely and accurate manner.

Knowledge Translation:

  • Analyzes, appraises and synthesizes research and best practices to use as a foundation for enhancing the delivery of quality patient care.
  • Uses knowledge translation techniques in innovative ways to put research-based knowledge into daily practice and to support and educate other members of the health care team. Contributes to the dissemination of nursing knowledge through presentations and publications.

Scholarship:

  • Evaluates outcomes of practice, practice patterns and systems of care against local and national benchmarks to identify variances in practice outcomes and population trends.
  • Identifies issues related to clinical practice and patient care and develops innovations to improve care and care delivery for patients, families, and communities. Designs, directs, participates in and evaluates quality improvement methodologies to promote safe, timely, effective, efficient, equitable, and patient-centered care.

Leadership:

  • The NP acts as a transformational leader by using ideals, inspiration and intellectual stimulation to influence the behaviours and attitudes of others towards the goal of excellent patient care. Acts as a consultant and resource to members of health care team and external groups in the process of decision-making strategic planning and problem solving by committee involvement, participation in program development and evaluation.

Key Qualifications

Minimum masters level nurse practitioner preparation

  • Adult or Primary Health Care NP (Registered Nurse in the Extended Class) in good standing with the College of Nurses of Ontario Master’s degree in Nursing with specialized clinical focus in Nurse Practitioner Practice.
  • Completion of education to prescribe controlled substances required by the College of Nurses of Ontario or equivalent.
  • Advanced assessment and diagnostic reasoning skills
  • Excellent communication skills and the ability to develop positive relationships with patients, families, physicians and inter-professional team members
  • Experience with research methodology, program planning and evaluation
  • Current Basic Cardiac Life Support certification
  • Satisfactory performance and attendance required
  • Demonstrated experience and leadership in education and research.
  • Current experience and demonstrated success as a Nurse Practitioner Practice in infusion therapy, women’s health, or hematology preferred. Demonstrated and comprehensive understanding of symptom management related to this patient population is required.
  • Ability to function as an independent practitioner and a collaborative team member.
  • Membership in the Nurse Practitioners’ Association of Ontario (NPAO) or Registered Nurses’ Association of Ontario is strongly preferred.
  • Excellent team-building, negotiation and conflict resolution skills.
