Position Overview
The Community Rehab Worker is responsible to support clients suffering from severe and persistent mental health disorders in their path towards the optimal functional and occupational state. While promoting the psychosocial rehabilitation approach and supporting recovery, the interventions will be held in the client’s home environment. The Community Rehab worker also supports the client’s development in his or her various rehabilitation and recovery functions: personal care, productivity and leisure, interpersonal relationships, etc. so that his or her quality of life is maximized.
In an ACTT setting, the Community Rehab worker also assumes a caseload for the functions of admitting, evaluating, planning treatment and interventions, educating, community networking and other administrative functions related to the services offered to the client. The vocational worker plays an important role regarding clients’ objectives relating to work, educational and volunteering activities

The Assertive Community Treatment Team (ACTT) is based in Casselman, ON
Salary of Tech 5 as per O.P.S.E.U. Collective Agreement.

What we are looking for
- College degree in health, human sciences or related field to mental health
- Minimum of three (3) years’ experience in Mental Health and/or
Concurrent Disorders.
SKILLS / COMPETENCIES:
- Intervention techniques and case management with clients suffering from severe and persistant mental health issues and concurrent disorders.
- Experience in providing individual support (problem solving,
role play, modeling and support).
- Ability of intervening and supporting clients in their vocational goals.
- Familiarity with the laws and regulations pertaining to mental health
of mental health and concurrent disorders interventions.
- Familiarity with the programs and community resources in
mental health and concurrent disorders.
- Great teamwork abilities and skills in working in collaboration with the various community services, more specifically those involve employment and/or studies.
- Knowledge of the ACTT model and functioning
- Ability to identify and use clinical judgement in at-risk situations.
- Read and understand complex documents such as psychiatric assessment reports and clients’ social history.
- Ability to write progress notes, transfer clinical information, assist clientele with various steps including filling out of
forms, prepare treatment plans and facilitate workshops.
- Good communication skills in French and English, both oral and
written.
- Availability to work evenings.
- Must possess a valid driver’s license and have access to a
vehicle to provide services in the community.
