Job Description

Position Summary

Reporting to the Infection Prevention and Control Lead (IPAC Lead), the Part-time IPAC Lead, coordinates and implements with the IPAC Lead, the IPAC program and processes of the Nursing Departments

Qualifications and Skills Required

  • Completion of RN/RPN degree or diploma, or Bachelor’s degree in other healthcare related field, e.g., Microbiology, Epidemiology, Public Health, Health Science, Medicine or Occupational Health

  • Current certification to operate as a Registered Practical Nurse or Registered Nurse in Ontario who is a member in good standing with the CNO

  • Certification in Infection Control (CIC) is an asset or completion of an IPAC Canada endorsed course and certification in IPAC within 18 months of hire with current demonstrated experience with best practice guidelines for infectious disease prevention and control

  • A minimum of 3-5 years’ work experience as a RPN or RN in the Long‐Term Care sector

  • Have good understanding of adult learning principles and teaching methods to facilitate learning

  • Demonstrate leadership and communication skills

  • Thorough knowledge of applicable Long-term Care legislation

  • Familiarity with Point Click Care an asset

  • Current First Aid/CPR

  • Ability to work as a member of a team and independently

  • Cultural sensitivity to the unique ethnic and faith community

  • Knowledge of Montessori Philosophy

  • Proficiency in Microsoft Office (Word, Excel, PowerPoint, and Outlook)

  • Two-step tuberculosis test obtained within the last six month as well as MMR vaccination proof

  • Fully vaccinated against COVID-19, including booster

  • Current (within 6 months) Vulnerable Position/ Sector Screening (VPS)

  • Legally entitled to work in Canada

Key Responsibilities

  • Part-Time Infection Prevention Control Lead of the long-term care home, reporting directly to the IPAC Lead
  • The expectation of the IPAC Lead role, shall be 26.25 hours per week at Pleasant Manor and 26.25 hours per week at Tabor Manor (eg Part-time IPAC - 2 days at PM 1 on week 1 and 3 days at TM 2 week 2, remaining hours covered by the Full-time IPAC Lead)
  • Works in collaboration with the Director of Care/Director of Clinical Services to monitor surveillance activities and manage outbreaks in the Home.
  • Convening the Outbreak Management Team (OMT) at the outset of an outbreak and regularly throughout an outbreak.
  • Be on 24-hour call for consultation and /or reporting purposes as required.
  • Works together to ensure IPAC program compliance and implementation.
  • Coordinate and implement IPAC practices in collaboration with Public Health to meet legislative and regulatory compliance.
  • Ensure surveillance and screening programs are in place for residents and team members.
  • Consults with the physicians/nurse practitioners on laboratory results and trending of data.
  • Collaborates with the nursing team on the clinical management of residents based on infection.
  • Ensure IPAC audits as per legislative requirements during outbreaks and complete trends analysis on daily basis.
  • Leads and ensures the coordination of annual flu clinics and immunization programs.
  • Ensure a hand hygiene program in accordance with any standard or protocol issued by the MLTC.
  • Manage and oversee the IPAC program and delivery of its education to all staff, caregivers, volunteers, visitors, and residents.
  • Ensures the IPAC program is operated efficiently and in compliance with the Long-Term Care Act and Regulations
  • Complete monthly/ weekly health and safety inspections for IPAC and safety practices
  • Evaluate and update the outbreak management system.
  • Working with interdisciplinary IPAC team to implement and manage IPAC program.
  • Educate and audit staff, caregivers, volunteers, visitors, and residents on the IPAC program.
  • Complete mask fit testing for new hires and annually for existing staff.
  • Order vaccinations, store and monitor the fridge temperatures.
  • Prepare and Organize vaccination clinics.
  • Other duties as assigned by the Infection Prevention and Control Lead (IPAC Lead).
