Client Care Intake Manager full time position

  • Job Type & Duration: Permanent, Full-time
  • Salary Range: $68,000 to $70,000 yearly
  • Shift Information: Monday to Friday, 37.5 hours per week
  • Number of Positions Open: 1 new vacant position
  • Posting Period: November 21.2025 to January 21.2026

The successful candidate’s areas of responsibility will include, but are not limited to, the following:

  • Reviewing and speaking with prospective clients during the intake process, reviewing services, differentiators and scheduling assessments.

  • Conducting assessments ; review care needs and review supplies needed

  • Proactive active care management for clients to include regular visits, management of our care plans and support for our client needs.

  • Ensuring that our caregivers are adequately trained to deliver client care as outlined in the care plan.

  • Monthly reporting as directed by Supply Ontario

  • Initial point of contact for Supply Ontario

  • Respond to medical emergencies

  • Providing extra support for new client cases, including home visits within the first two weeks of service, to ensure complete client satisfaction.

  • Working with team members to ensure high quality service delivery and ongoing client and employee satisfaction.

  • Providing after hours support for our clients, referral partners and prospective clients. Driving to facilities to support and review care plan.

  • Actively follow up with potential and active contacts in our community.

  • Maintain a record of interactions in Well Sky.

  • Participate in caregiver training and orientation. Support field staff, on updating care plans and review any issues.

  • Experience in identifying patients with complex needs, and providing routine and
    emergency care, health teaching, and counseling to patients with defined needs and/or
    predictable outcomes.

  • Developing, monitoring, and updating detailed care plans, as assigned.

  • Directing the wellness staff and ensuring their responsibilities are completed in an accurate and timely manner.

  • Providing medication support and administration.

  • Responding to all emergency. Evaluate and assess patients' responses to planned health care interventions; identify patients with complex needs; accepting and transferring patients; contribute to the development of treatment and care plans.

  • Completing pertinent reports correctly and in keeping with assigned deadlines.

  • Meeting the Standards of the College of Nurses of Ontario at all times.

  • Meeting the Ministry of Labour documentation requirements

  • Work Monday to Friday day shift may vary depending on business needs

  • Rotation of On-Call evenings and weekends every 5 week, as needed

  • Educating training for employees

  • All other duties, as assigned

Other key qualifications for this position will include, but are not limited to:

  • Current certificate of competence with the College of Nurses of Ontario .
  • Over 5 years of nursing experience in working in Health Care.
  • Valid First Aid and CPR certificate is preferable.
  • Strong communication and customer service skills
  • Leadership skills, with a positive attitude
  • Able to solve problems, with a high emotional intelligence
  • Ability to accurately follow written and verbal instructions.
  • Computer skills
  • Ability to communicate to our residents, their families, colleagues, etc., in a friendly and professional manner.
  • Comfortable with building business relationships and community engagement
  • Effective time management skills to work under specified time constraints and in accordance with job routines, while observing policies and procedures as well as applicable work standards and safety regulations.
  • The ability to focus on multiple tasks in a fast-paced work environment.
  • Must be able to anticipate problems and use sound judgment and tact in handling confidential matters.
  • Vulnerable sector screen, and satisfactory professional references.
  • A valid driver’s license and access to a reliable vehicle
