##Job Description

JOB SUMMARY

In Adult Day Programs, the Coordinator is responsible for various aspects of day-to-day operations for the program including; client referral, admission, on-going service planning, discharge, month-end and administrative requirements. The ADP Coordinator provides daily support and functional direction to the staff team working in a supportive group environment to offer standard recreation programming to seniors living in the community to stay connected, to be active and to keep learning.

The ADP Coordinator is the lead role in client service planning with clients, care partners, and staff to continuously support client need and determine ongoing suitability.

The ADP Coordinator works collaboratively with Seniors Community Program staff and other community agencies to connect clients to additional services that support client independence.

QUALIFICATIONS

EDUCATION

  • Diploma or degree in Recreation & Leisure Studies, Gerontology or Kinesiology or equivalent, from an accredited post-secondary institution
  • Valid First Aid Certification is required
  • Certification in the SAM S.T.E.P.S. model of exercise is preferred
  • Completed courses in DementiAbility, GPA or geriatrics preferred
  • Safe Food Handling Training is preferred
  • Certificate in Adult Education is an asset

KNOWLEDGE/EXPERIENCE

  • Minimum 1- 2 years of recreational/social program coordination experience within a community-based environment program serving a geriatric population; or equivalent relevant experience
  • 1-2 years demonstrated experience leading exercise for seniors programs
  • 1-2 years experience working in senior settings with a focus on development, implementation, and evaluation of recreation content and program plans, and the ability to adapt programming and services to community and client need
  • Demonstrated knowledge and experience in determining suitability and supporting the transition to alternate services within the scope of the Adult Day and Seniors Community Programs
  • Knowledge of relevant community resources for seniors
  • Knowledge of client safety issues and demonstrated capacity to apply client safety practices
  • Knowledge of diverse senior populations experiencing cognitive and physical challenges related to aging in a supportive environment

  • Demonstrated leadership, collaboration and coordination skills
  • Excellent organizational, time management and planning skills
  • Excellent communication skills, both written and verbal
  • Ability to work collaboratively as part of an interdisciplinary team in a respectful manner
  • Enthusiasm, flexibility and ability to cope with change
  • Developing themed programming content based on approved themes
  • Must have excellent rapport with people and demonstrated working ability with Seniors
  • Demonstrated knowledge and ability to use relevant internet & computer applications (e.g. Client Record Management System (CRMS-database), Microsoft Suite applications (Word, Power Point), online meeting & collaboration applications (e.g. Zoom, Teams, etc), applications such as Canva and Activity Connection (or similar)

SPECIAL CONDITIONS

  • In accordance with the Corporate Criminal Record Check Policy, the position requires the incumbent to undergo a Criminal Records Check and submit a Canadian Police Clearance Certificate including a vulnerable sector search.
  • Compliance with any requirements, including immunization and screening measures, of the infection prevention and control program as required by applicable legislation/Act, regulation or policy; some measures are only recommendations (not requirements)
  • Must be physically and mentally capable of performing the duties of the position,
  • Must have reliable transportation and be able to work at various program locations throughout Niagara, based on client need/management discretion
  • Must have a good attendance record
  • For positions assigned to an identified French Language Services (FLS) location, must be fluent in French (both Written and Verbal)

##Shift Information

Currently scheduled 40 hours days shifts, may be required to work days, evenings and nights including weekends.
