About the Role:

Reporting to the Director of Quality Assurance the Facilitator will work with the Quality Team in offer person directed services (including PDPs) to the supported person and his/her support network. Identifying and assisting in developing person’s goals and dreams for the future and articulating a plan to achieve is a key focus. Personal plans of action are developed to assist each supported person (and team) to achieve the chosen quality of life goals. The Facilitator shall work with support circles of persons supported residentially across each Kerry’s Place region to ensure that Person Directed Plans are offered and that action plans are documented. This position, based in Newmarket, supports all Kerry’s Place regions.

  • To be a positive role model and consistently engage in practices that reinforce the Person Centered Approach, Mission, Vision, Values, Principles, Policies and Procedures of Kerry's Place Autism Services.
  • Adheres to Employee Responsibilities for Health & Safety as defined by the Occupational Health and Safety Act (OHSA), including immediate reporting of workplace injuries and/or hazards to a member of Kerry’s Place management, and use of any personal protective equipment provided.
  • Participates in ongoing Training and Development and ensures all mandatory training requirements are met and maintained.
  • Responsible for the implementation of quality outcomes initiatives, activities and organizational development to achieve the goals set out by Kerry’s Place and persons supported.
  • Promote and support the use of a person-directed approach for supported persons in all meetings and initiatives focused on program development and continuous quality improvement.
  • Coordinate support circle meetings for persons supported within supported living environments across Kerry’s Place regions.
  • Develop and encourage community links.
  • Ensure that the supported person is central and appropriately engaged in the planning process.
  • Ensure that planning recognizes and is sensitive to the supported person’s cultural

background.

  • Ensure that accurate notes of support circle meetings/action plans are distributed to support circle members and are inputted into electronic database.
  • Explore with the supported person and support network how to best support the person in making effective connections that will help them move closer to their goals.

Education Qualifications:

  • Post-secondary education, primarily in the field of social services.
  • Training in person directed approaches, facilitated approaches to meetings, conflict resolution or equivalent is an asset.

Knowledge and Experience Requirements:

  • Knowledge of Kerry’s Place Policies/Procedures relative to services and support would

Be considered an asset.

  • Maintains a working knowledge of QAM and policy directives and any other relevant legislation.
  • Knowledge of community resources.
  • Experience/skills in network development and connecting people to informal and formal resources.
  • Strong communication and interpersonal skills.
  • Proven skill and experience with team building.
  • Proven ability to create a well written plan.
  • Ability to manage a large caseload.
  • Strong active listening skills (attention to verbal and non-verbal cues).
  • Ability to engage people respectfully with creativity.
  • Demonstrated proficiency with computer skills and good working knowledge of Microsoft software.
  • Ability to work independently.
  • Maintaining flexibility in dealing with a fluctuating workload.
  • Ability to travel across Kerry’s Place regions (may include overnight travel).
  • French language considered an asset.

Working Conditions/Physical Requirements:

  • Must have the ability to complete the Non-Violent Crisis Intervention (NVCI) certification, First Aid/CPR as well as perform the required physical techniques. Training will be provided at no cost
  • A valid Driver’s License, with a minimum G2 and access to a reliable vehicle is required and proof of third party liability insurance coverage
  • As a condition of employment, employees must be able to provide a Driver’s Abstract in good standing with fewer than three demerit points
  • Ability and willingness to work flexible hours including evenings and weekends

Welcome to Kerry’s Place Autism Services. We are Canada’s largest service provider to families with children, adolescents, and adults, with Autism Spectrum Disorder.

Since 1974 Kerry’s Place has been creating, and providing evidence-based supports and services across the province of Ontario that enhance the quality of life of those with ASD.

Today, we are envisioning a future where all persons with ASD are participating fully in their communities.

With staff numbering more than 1200, Kerry’s Place is one of the only organizations in the country that serves the entire life span of those with ASD.

Through the Ontario Autism Program, we provide children and youth - and their families - a menu of timely, evidence-based services needed to achieve their goals at home, at school, and within their communities. This initiative serves individuals with ASD up to age 18, delivering skill-building groups, individual consultation, and family support.

We provide numerous Community Services and Supports, including our day and overnight respite options to provide a well-deserved break for families with loved ones with Autism, along with our many camps and leisure groups.

Our Adult Skill-Building and Peer Support Groups broaden the social circle for every individual we support. Our Employment Programs lend assistance to young adults with Autism to help develop skills that lead to meaningful employment opportunities.

We provide adults with ASD Residential and Semi-Independent Living Supports at 90 distinct properties across Ontario. Our group living approach provides a safe, 24 hour, home environment where residents are encouraged to pursue their own personal goals including outings into the community, employment, and an array of interests. Many of our residence grow with Kerry’s Place, spending upwards of 30 years with us. And through a lifetime of shared pursuits and generational experiences, we too, grow with them.
