REPORTS TO Elite Support Services Manager
MAIN OBJECTIVE Support community involvement, daily life skills activities, and promote individuality. Increasing quality of life, communication and overall well-being.

DUTIES AND RESPONSIBILITIES
The essential duties and responsibilities of this position include, but are not limited to, the following:

Annual development and implementation of individual service plans in alignment to assessed needs and individual goals
-
Development and adherence to established daily activity schedule in alignment to the Individual Service Plan. Activities to be considered: life skills, fitness classes, health and wellness, community and recreational outings, Snoezelen relaxation room, movies, dance, yoga, art/ music therapy.

Plan, prepare, implement, evaluate and update individuals’ program

Maintenance of daily observation and communication records
-
Privacy and security of confidential information safeguarded
-
Compliance to Health and Safety Procedures and reporting potential hazards/ safety concerns to management
-
Actively engaged in clients progress and participation in activities
-
Daily activity and progress records for individuals are completed and accurate
-
Timely completion of documented incident reports to management
-
Reporting of all medical concerns such as: illness, injury, communicable diseases
-
Adherence to open, clear, respectful, and positive communication to clients, coworkers, and management
-
Immediate reporting of all forms of harassment or misconduct to management
-
