The Quality, Safety and Patient/Family Experience portfolio at the Centre for Addiction and Mental Health (CAMH) is seeking two (2) full-time, permanent Quality and Patient Safety Specialists reporting to the Manager of Patient Safety. The successful candidate will provide leadership in advancing the safety culture at CAMH, and supporting the organization-wide patient safety program

The Quality Patient Safety Specialist will be required to:

  • Facilitate and coordinate various patient safety and quality initiatives, corporate quality improvement projects and other strategic priorities
  • Support the development of quality indicators and standards to assess the quality of care provided
  • Oversee administration of the CAMH incident reporting system (RL Datix)
  • Utilize clinical knowledge, skill and judgment to identify patient safety themes/trends through daily review and analysis of incident reports
  • Follow-up on patient safety events and Good Catch/Near Misses reported via the incident reporting system, and facilitate the incident analysis process for patient safety events
  • Collaborate with clinical teams in proactively identifying potential patient safety and quality priorities, and facilitate problem-solving in consultation with appropriate team members/stakeholders
  • Use clinical competencies and frameworks to implement tools and methods that advance safety and quality (e.g. Root Cause Analysis, Failure Modes and Effects Analysis, Lean, etc.)
  • Support and encourage incident reporting throughout the organization focusing on ensuring a ‘Just Culture’ framework
  • Serve as a quality and patient safety resource to all program/services/departments
  • Develop curriculum, provide education and deliver ongoing training to staff on quality improvement and patient safety methodologies
  • Collaborate with healthcare teams to implement and support quality/patient safety improvement initiatives.

Job Requirements

The successful candidate will have the following qualifications and experience:

  • Master’s degree in a healthcare-related field required
  • 3-5 years of clinical experience
  • Registration with a health professional regulatory college
  • Experience in patient safety, risk or quality improvement preferred
  • Formal training and/or certification in patient safety would be an asset
  • Proven education and facilitation skills, ability to train and motivate staff; effectively leading improvement projects and process redesign
  • Exceptional interpersonal skills (developing and maintaining positive relationships, communicating clearly and effectively with people at all levels of the organization, working with others to identify, define and solve problems)
  • Strong analytic abilities (including problem recognition and problem solving)
  • Experience with MS Office (i.e. PowerPoint, Visio, Excel, etc.), RLDatix ©, data analysis, data manipulation, statistics and report generation
  • Experience preparing detailed reports, publications and presentations
