Highlights Supports the development, implementation, and evaluation of person-centred planning and services that focus on helping people find or build toward paid employment, make new connections and friendships, strengthen existing relationships, learn and grow in the areas of their interest, and develop increased independence, within the community of their choosing. This support is informed by a deep understanding of the person’s needs, strengths, capabilities, associated risks and safeguards, and opportunity areas related to the person’s intentions for their life.

Duties And Responsibilities Discovery Process: Lead individuals through a structured discovery process that includes exploration activities, observations, and interviews to gather in-depth information about their dreams, goals, strengths, and interests.

Goal Co-Creation: Work collaboratively with individuals to establish personalized goals in areas such as learning, inclusion, friendship, and employment. These goals are designed to align with each individual’s unique aspirations and needs.

Support Strategy Development: Develop and implement intentional support strategies tailored to each individual. These strategies focus on increasing confidence, independence, and the ability to build and sustain natural supports within the community.

Regular Check-Ins: Conduct consistent check-ins with individuals to assess progress toward goals, evaluate the effectiveness of support strategies, and make necessary adjustments to ensure continuous improvement and success.

Research and Indirect Support: Engage in behind-the-scenes support activities, such as researching community resources, identifying potential social and employment opportunities, and establishing connections with local organizations and employers.

Supported Employment: Apply supported employment principles to assist individuals with job preparation, job searching, and sustaining employment. This includes networking with employers, providing job coaching, and supporting individuals in developing necessary employment skills.

Networking and Community Engagement: Actively network with employers, community partners, and other stakeholders to facilitate a broad support network, access to resources, and opportunities.

Record Keeping: Maintain thorough and accurate records of all activities, including discovery findings, goal-setting sessions, support strategies, progress reports, and communications. Ensure that all documentation is up-to-date and reflects the individual’s progress and any adjustments made to support strategies.

Professional Collaboration: Participate in weekly team meetings to share insights, align on strategies, and contribute to a cohesive team approach. Collaborate with other members of the support team to ensure that support provided to individuals is well-rounded and integrated with broader team efforts.

Reports To Other related duties as required.

Rate Of Pay Inclusion Facilitator: $27.15/hour to $30.95/hour

Probation 3 months

Driving Requirement Preference will be given to applicants with Class 5 Driving License

Preferred Training Or Experience Post-secondary certificate, diploma or degree in social sciences preferred Education in a related discipline such as human/social services, community support and/or rehabilitation services or equivalent combination of education/training and Supported Employment experience.

This position requires strong interpersonal skills and excellent written and verbal communication.

Ability to listen deeply to the goals and life expectations of the person supported.

Highly self-motivated, and able to work independently and as part of a team.

Understanding of a case management approach.

Strong organizational and time management skills.

Understand and apply the principles of person-centredness and self-determination.

Knowledge of the challenges experienced by individuals who face barriers or exclusion due to disability.

Nonjudgmental and genuinely positive attitudes towards all community members.
