About the role
Two Rivers Family Health Team (TRFHT) is seeking a part-time 2-3 days per week (0.4-0.6 FTE) Health Promoter to cover a parental leave. Contract is available for up to 18 months. TRFHT provides primary health care including chronic disease management, health promotion and disease prevention to a population of patients in the local community and surrounding area. TRFHT employs an interprofessional team of health care providers who work in a collaborative model of care.
POSITION SUMMARY:
The Health Promoter will collaborate with team members to support the delivery of preventive primary care and promote the health and well-being of clients and the broader community. This role will involve assessing health needs, planning, coordinating, and providing services for specific populations, along with their families and caregivers. The Health Promoter will play a key role in developing, implementing, and evaluating wellness, lifestyle, and chronic disease management programs. They will also engage with community partners and conduct systematic reviews to identify existing programs and explore new opportunities. By sharing their expertise in health promotion and disease prevention, the Health Promoter will help connect individuals with appropriate services and healthcare professionals.
QUALIFICATIONS:
- Bachelor’s degree in health or a related discipline from an accredited university (e.g., Health Sciences, Health Promotion, Nursing, or a similar program)
- Master’s degree with a focus in Health Promotion, or a related field is considered an asset
- Smoking Cessation certification, TEACH program would be an asset
- Preferred qualifications include certification and hands-on experience in the development and delivery of Self-Management models focused on health promotion, disease prevention, and chronic disease management
- Minimum of 1-3 years’ proven experience in a primary health care (family medicine) or community-based setting is preferred
- Strong understanding of client-centered care, change management principles, community service delivery models, short-term treatment approaches, and chronic disease management frameworks
- Demonstrated ability to provide individual and group-based education and counseling using motivational interviewing techniques
- Excellent organization skills, with an ability to set priorities and work effectively in a fast-paced environment
- Highly effective decision making, problem solving and organization skills combined with interpersonal and communication skills both oral and written
- Be able to work collaboratively within an interdisciplinary health care team, but also be self-directed and able to work independently
- Demonstrate a high level of competency with computers and information technology
