Outreach Workers are responsible for providing community support to clients of the Mental Health Program, which enables the client to maintain an optimal level of functioning while living with a chronic mental health illness. Outreach Workers work in an interdisciplinary team setting.

REQUIREMENTS:

Minimum two (2) year Post Secondary School Diploma in Social Service Work
-
Previous experience working with individuals with severe mental illness in Renfrew County and in a community setting
-
Experience in a team-oriented environment an asset
-
Valid G Driver’s License (or equivalent)
-
Has an available, safe vehicle, and clean driver’s license record three (3) years prior to date of hire
-
Carries appropriate automobile liability insurance of no less than $2,000,000
-
Demonstrated ability and experience facilitating life skills and social recreation groups
-
Excellent interpersonal and conflict resolution skills
-
Excellent verbal and written communication skills
-
Knowledge of Renfrew County
-
Excellent organizational skills and ability to set priorities
-
Ability to work collaboratively as a team member
-
Ability to coach, mentor and influence
-
Staff may be cross-trained and support other function centres in a time of need
-
Proficiency in the use of computer applications and software programs
-
Embodies the LEAN ideology by actively and continually seeking opportunities for continuous improvement.

Must have demonstrated ability to meet the attendance standards of the Hospital

Bilingualism preferred (French/English)

Located approximately 150 kilometres west of Ottawa, the Pembroke Regional Hospital delivers a broad range of acute, post-acute, outpatient and diagnostic services to a mixed urban and rural population of approximately 55,000 residents in the City of Pembroke, the Town of Petawawa, and surrounding municipalities.
