Days & Evenings: 0700-1500/1500-2300

SALARY: $32.84– $44.96 Per Hour

SUMMARY:

Under the direction of the Manager, or designate, and in collaboration as a member of a multidisciplinary team, the Licensed Practical Nurse (LPN) works collaboratively with the care team to support residents in establishing and maintaining a pleasant, safe and homelike atmosphere by providing nursing services and interventions to residents based on assessment and care planning procedures. The LPN co-ordinates health services, and provides support in activities of daily living, including administering medication.

The LPN functions as an independent practitioner and accesses the Registered Nurse (RN)/ Registered Psychiatric Nurse (RPN) or Clinical Supervisor for collaborative support and/or clinical guidance in situations that fall outside the scope of an LPN, such as increased resident acuity or care. The LPN operates in accordance with the competency guidelines and scope of practice within the Standards of Practice as outlined by the British Columbia College of Nurses and Midwives, and in accordance with organizational operating policies and standards and department specific protocols.

What You Will Do

Key Responsibilities:

  • Provides nursing care to assigned residents following established policies, procedures, and individualized care plans.
  • Performs head-to-toe assessments, monitors changes in condition or lab results, and communicates concerns to the RN/RPN.
  • Participates in the development and updates of care plans and contributes during team meetings and family conferences.
  • Administers medications and monitors for effectiveness or adverse effects, reporting as needed.
  • Assists with admissions, transfers, and discharges in line with organizational protocols.
  • Assesses residents’ physical, emotional, social, cultural, and spiritual needs and encourages independence in daily living.
  • Documents all care activities, assessments, and resident updates accurately and promptly.
  • Supports onboarding of new staff and students by demonstrating procedures and sharing knowledge.
  • Participates in quality improvement activities, staff meetings, and educational sessions.

What You Bring

QUALIFICATIONS

Education, Training, and Experience:

  • Graduate of a recognized program for Practical Nurses.
  • Registration with the BC College of Nurses and Midwives (BCCNM) as a practicing LPN Registrant.

Skills and Abilities:

  • Demonstrated knowledge and competencies to work at full scope of practice as currently required by the BCCNM.
  • Communicate effectively, both verbally and in writing, with residents, families, co-workers, members of the multi-disciplinary care team and the public.
  • Ability to deal effectively with others.
  • Ability to organize work and implement a plan of care.
  • Effectively model behaviours associated with the Home for Us philosophy of care and mentor staff in the same.
  • Ability to operate related equipment.
  • Physical ability to carry out the duties of the position.
