Position Summary
The Continuing Care Program is seeking a Full-time PCCT Navigator/Clinical Resource Nurse to join their team. This position demands an exceptional level of nursing practice expertise as well as the personal ability to effectively coach, teach and nurture the staff being supported.
The PCCT Navigator/Clinical Resource Nurse is responsible (but not limited to) the following:

Facilitating effective resource utilization coordinating patient care resources and patient flow on a daily basis for palliative unit. This also includes outreach coordination with the Palliative Care Community Team (PCCT)

Supporting front line clinical skills and professional development by being a mentor, coach and ready resource for staff

Coordinating the mandated data collection and submissions, (related to financial resources)

Supporting corporate projects and front line staff education on appropriate topics. This includes a strong lead and facilitation role in the discharge transitions for palliative patients.

Essential Qualifications
- BScN
- Registered Nurse in good standing with the College of Nurses of Ontario
- Adult Educational Principles and experience delivering adult education
- Teaching experience in classroom and clinical
- Current CPR certification, on hire
- Current certification in Palliative Care or Geriatrics with the CNA or a commitment to obtain within agreed time frame
- Current CPI certification
- Current IV Therapy/Medications
- Physical Assessment skills
- Working knowledge of Microsoft Office, database management, healthcare research
resources and search engines
- Demonstrated experience and expertise in Palliative Care
- Demonstrated experience and expertise in Chronic Disease, Senior Care, Patient Falls experience and Elder Abuse
- Strong clinical skills
- Experience leading projects and training including community linkages
- Experience working with a well functioning interprofessional team
Preferred Qualifications
- MScN preferred
- Working knowledge of RAI-MDS and/or NRS (FIM) data systems
- Working knowledge of UKG scheduling/staffing software
- Continuing education in managing responsive behaviours, i.e. Montessori, P.I.E.C.E.S
- Competence in EPIC ClinDoc (training can be provided but must pass)
Hours of Work
Monday to Friday 8 hour day shift. Shifts, hours and days required to work may change according to departmental requirements and as per the ONA Collective Agreement.
