Waypoint Centre for Mental Health Care is a 315-bed academic and teaching hospital that provides specialized mental health, addiction and geriatric care. Located on the shores of Georgian Bay, we serve some of the most complex and disadvantaged individuals in Ontario and are the sole provider of high-secure forensic mental health services in the province.
The North Simcoe Muskoka Specialized Geriatric Services (NSM SGS) Program is a regional program under the leadership of Waypoint Centre for Mental Health Care. The NSM SGS program was established in 2016 to advance the care of frail older adults in the NSM region and their caregivers. With a mandate for leadership, clinical service, education and mentorship, research and ethics as well as advocacy, the program collaborates with a broad variety of partners from within and outside the NSM health system to improve patient outcomes, build capacity and foster system change.
The Clinical Manager of NSM SGS Central Intake is currently seeking a Registered Practical Nurse for a permanent full time role working Monday to Friday business hours. This position will provide clinical intake/priority assessment for referrals and system navigation for seniors living with complexity, their health care providers, and care-partners.

KEY ACCOUNTABILITIES:
- Assess client condition and needs; develop and implement therapeutic client-centred service/navigation plans.
- Actively participate in the identification and coordination of provision of services and resources for clients including:
- Confirming reason for referral
- Identifying actual and potential client health and safety risks
- Ensuring eligibility for service(s).
- Determining service(s) accepted to.
- Determining priority level for clinical team(s).
- Identifying additional services/resources that will promote primary/secondary prevention for geriatric syndromes.
- As required, call client, referral source and/or family provider to confirm service needs, expectations and system navigation.
- Ensure client needs are met in the most efficient and effective manner, including managing waitlist, processing referrals in a timely manner and navigating and/or re-directing referrals to appropriate services.
- Assess, record, and report client condition through Meditech and other sources.
- Provide system navigation and geriatric syndrome education/support to seniors living with complexity, their health care providers and care-partners as required for non-registered clients.
- Contribute to a safe, therapeutic environment through awareness of the College of Nurses of Ontario Standards, program/facility wide policies and procedures, and mandatory training.
REQUIREMENTS/QUALIFICATIONS:
- Current registration in good standing as a Registered Practical Nurse with the College of Nurses of Ontario
- Recent experience working in a specialized geriatric services program is required
- Recent experience working in a centralized referral service is required
- Recent experience providing care for older adults living with complexity/frailty is required
- Demonstrated knowledge of and/or experience with services within the NSM region for older adults
- Strong computer software skills
- Demonstrated reasoning and analytical skills
- Results oriented work ethic to work independently and undertake tasks needed to accomplish work objectives and deliver quality, consistent and timely results
- Demonstrated leadership, conflict resolution, interpersonal and organizational skills
- Proven written and verbal communication skills
- Proven ability to adapt readily to change and reflects an optimistic and positive attitude
- Models and promotes core ethical practice, Waypoint Values (Caring, Respect, Accountability, Innovation)
- Preference for proficiency in French/English language skills.
