About the role

Position Overview/Summary:

The Primary Care Transformation Specialist will report to the Manager, Health System Planning, and work closely with the Mid West Toronto Ontario Health Team (MWT-OHT) Secretariat, the Family Practice Network co-chairs, and the emerging Interprofessional Primary Care Teams (IPCTs). The incumbent will be an integral part of a close-knit team of strategic health care professionals with a broad range of experience and skillsets. Specifically, the Primary Care Transformation Specialist will be responsible for supporting project management, coordination and implementation of OHT-wide projects specifically related to Primary Care Access and Attachment.

Duration: 1 Year contract

Salary Range: $71,000 - $85,000

Responsibilities:

Project Management:

• Co-design and oversee the implementation of a shared, centralized intake process to streamline attachment to primary care.

• Support the implementation of a supported intake model within practices to effectively manage the patient onboarding process, to reduce administrative burden and improve workflow.

• Support patient navigation for individuals awaiting attachment to primary care by assisting with identifying and accessing appropriate services during the interim period.

• Lead the MWT-OHT reporting efforts against key attachment metrics and support partners in their contribution to reporting.

Stakeholder Management:

• Be the point of contact with partner clinics to monitor attachments, assess clinic capacity, and address issues related to attachment.

• Oversee the development and execution of accountability agreements, ensure clinic compliance, and manage the flow of funding linked to attachment initiatives.

• Support the coordination of interprofessional shared resources across partner organizations by facilitating communication, alignment of standardized protocols, and tracking resource utilization to help optimize patient attachments.

• Engage with the Family Practice Network, Primary Care Access and Attachment Working Group, and partners to support the coordination and development of future IPCT funding applications.

Administrative Responsibilities:

• Support the decision-making and business planning of the OHT, specifically as it relates to the primary care access and attachment strategy.

Prepare timely, accurate and comprehensive briefing notes, project updates, and other executive documentation related to project planning, management and reporting, as required.

• Proven diplomacy and political acuity in navigating complex relationships and discussions.

Required Knowledge and Experience:

• 2+ years' experience in the healthcare industry.

• Excellent understanding of the Ontario health care system and primary care delivery models, and solid understanding of local health issues, priorities and needs.

• Comfortable in collaborating with diverse internal teams and external partners or stakeholders.

• Demonstrated commitment to and understanding of equity, anti-racism, and anti-oppression principles and practices is a strong asset.

• Demonstrated project management expertise.

• Experience with customer relationship management (CRM) systems.

 • Knowledge of patient attachment processes, centralized intake models, and interprofessional team-based care.

Required Professional Designation/Certificate:

Post Graduate Degree, preferably in Health Administration or Business Administration.
