Job Title: Registered Nurse (RN)

Status: In Person, Permanent, Full Time, 37.5 hours/week Monday to Friday (occasional evenings required)

Salary: $66,615 - $78,371

CoLFHT delivers comprehensive, team-based primary care designed to support our patients. As a Registered Nurse, you’ll be at the forefront of patient care, empowering patients through health education, prevention, and hands-on support.

Responsibilities:

  • Provide direct primary care to patients of all ages
  • Conduct nursing assessments, triage, treatments, and health education in accordance with CNO standards
  • Maintain accurate and timely documentation; participate in chart reviews, case conferences, and referrals
  • Administer immunizations and report per Ontario Immunization Guidelines
  • Stay current with nursing best practices and pursue ongoing professional development
  • Deliver direct primary care to patients of all ages, including wellness checks, well-baby visits, and injections
  • Support chronic disease management, preventative screening, and health promotion
  • Collaborate with physicians, nurse practitioners, and allied health professionals
  • Participate in program development and quality improvement
  • Opportunity to mentor and support interprofessional learners and students
  • Deliver care in clinic, community, and home settings
  • Lead infection control initiatives and report communicable diseases
  • Perform other duties as required to support evolving team needs

Qualifications

  • Diploma or Bachelor’s Degree in Nursing
  • Current registration in good standing with the College of Nurses of Ontario
  • Membership with the Registered Nurses Association of Ontario
  • Experience in primary care nursing (asset)
  • Proficiency with Electronic Medical Records (Telus PS Suite preferred) (asset)
  • Bilingualism (asset)
