Job Description The Registered Practical Nurse (RPN) provides safe, competent, and ethical nursing care to residents in accordance with the College of Nurses of Ontario (CNO) standards, the Fixing Long-Term Care Act (2021) , and the home’s policies and procedures. The RPN practices in collaboration with the interdisciplinary team and reports to the RN in Charge and/or Director of Care .

Key Responsibilities Resident Care & Clinical Practice

  • Provide direct nursing care to residents within the RPN scope of practice, including assessments, interventions, and evaluation of outcomes.
  • Perform ongoing monitoring of residents’ physical, cognitive, emotional, and behavioral status and promptly report any changes in condition to the RN in Charge.
  • Administer medications safely and accurately in accordance with physician orders, CNO standards, and home policies.
  • Provide wound care, treatments, and basic nursing procedures as per care plans and medical directives.
  • Support residents with BPSD, responsive behaviors, and psychosocial needs using person-centred approaches.

Electronic Documentation (PointClickCare – PCC)

  • Accurately document all nursing care, assessments, medication administration, and interventions in PointClickCare (PCC/eMAR/TAR) in a timely manner.
  • Complete head-to-toe assessments, progress notes, flow sheets, ADL documentation, and pain assessments as required.
  • Ensure documentation meets legislative, professional, and organizational standards and supports continuity of care.
  • Review and acknowledge alerts, tasks, and clinical triggers within PCC and escalate concerns appropriately.

Medication Management

  • Administer medications using eMAR and follow the Five Rights of Medication Administration.
  • Monitor for therapeutic effects, side effects, and adverse drug reactions and report concerns promptly to the RN.
  • Participate in medication reconciliation, narcotic counts (as applicable), and safe medication storage practices.
  • Initiate appropriate follow-up for medication refusals, omissions, or errors and complete required documentation and reporting.

Risk Management & Safety

  • Identify, assess, and report clinical risks such as falls, skin breakdown, infections, hypoglycemia, aspiration risk, and medication incidents.
  • Initiate immediate safety interventions within scope and notify the RN in Charge of any incidents or near misses.
  • Complete incident reports and contribute to follow-up investigations and corrective actions as directed.
  • Follow IPAC practices including hand hygiene, PPE use, isolation precautions, and outbreak protocols.

Communication & Collaboration

  • Communicate effectively with residents, families, and the interdisciplinary team regarding care needs and changes in condition.
  • Provide clear, timely verbal and written reports to the RN in Charge during shift handovers.
  • Collaborate with PSWs by providing guidance, delegation, and follow-up within the RPN role.
  • Participate in care conferences, shift huddles, and team meetings as required.

Regulatory & Policy Compliance

  • Practice in compliance with CNO standards, Fixing Long-Term Care Act (2021), Ontario Regulation 246/22, and organizational policies.
  • Maintain confidentiality and privacy of resident information at all times.
  • Participate in mandatory education, IPAC training, emergency preparedness, and quality improvement initiatives.

Professional Accountability

  • Recognize limitations in scope and escalate complex or unstable resident situations to the RN in Charge immediately.
  • Maintain current registration with the College of Nurses of Ontario.
  • Demonstrate professionalism, accountability, and ethical practice at all times.
  • Support a culture of safety, respect, and continuous improvement within the home.

Reporting Relationship

  • Reports directly to the RN in Charge and works under the direction of the Director of Care / Assistant Director of Care.

Compensation Placement in the salary range will be based on factors such as market conditions, internal equity, and candidate experience, skills, and qualifications relevant to the role.
